Abstract and Introduction
Abstract
Objective: Germline mutations in the tumor suppressor genes BRCA1 and BRCA2 predispose women to breast and ovarian cancer. Female carriers of BRCA1 or BRCA2 gene mutations have very high lifetime risks for breast and ovarian cancers. Genetic abnormalities occur in all cancers, so BRCA-related pathways are critical because they serve to safeguard genetic content. Although protecting genetic information is a general function, BRCA-related pathways seem largely specific to preventing breast and ovarian cancer. The objective of this study was to resolve this difference between the theoretical functions of BRCA genes and their specific clinical effects.
Data Sources, Data Extraction, Data Synthesis: The author collected data published in > 30 epidemiologic studies on the incidence of cancers other than breast or ovarian in mutation carriers and in large populations eligible for mutation testing. Data were extracted and used directly as published whenever possible with a minimum of statistical manipulation.
Conclusions: Although mutations target breast and ovary, a broader spectrum of cancers also occur with statistically significant elevated frequencies. Risks for "all cancers except breast or ovary" are elevated, with some population subgroups differing with regard to how frequently elevated risks were found at individual sites. Additional sites at risk included stomach, pancreas, prostate, and colon. The increased risk ranged from about 20% to 60%, with the greatest increases in risk in stomach and pancreas. The collected data show BRCA-pathway functions are probably required at multiple sites, not just in breast or ovary. Known interactions and relationships among BRCA-related pathways strongly support the idea that their inactivation provides growth or survival advantages for a variety of cancers. The data suggest applying an increased level of clinical alertness to those with defects in BRCA-related pathways. Identifying molecules that confer growth or survival advantages to BRCA-related cancers may provide broadly useful targets for chemotherapy or chemoprevention.

Las portadoras femeninas de las mutaciones en los genes BRCA1 y BRCA2 tienen riesgos de por vida muy altos para cáncer de mama y cáncer de ovario. Las anomalías genéticas ocurren en todos los cánceres, por lo que las vías relacionadas con el componente BRCA son críticas, ya que sirven para salvaguardar el contenido genético. Aunque la protección de la información genética es una función general, las vías relacionadas con BRCA parecen ser muy específicas en prevenir el cáncer de mama y el cáncer de ovario. Ensayo CURE
La presentación del ensayo CURE (Clopidogrel in Unstable angina to prevent Recurrent Events; Clopidogrel en la angina inestable como prevención de eventos recurrentes) ha sido una de las más comentadas, tanto por los asistentes a esta reunión como por los medios de comunicación. Este hallazgo, aunque todavía preliminar y cuya verificación se espera con impaciencia para su publicación, es bastante sorprendente y es probable que se incorpore con prontitud en la práctica clínica. Tanto es así que en la actualidad se coloca una endoprótesis vascular (stent) en muchos pacientes con un síndrome coronario agudo, y como norma tras esa intervención quirúrgica se administra clopidogrel durante 1 mes. La ticlopidina (Ticlid) también se ha usado tras la implantación de una endoprótesis vascular, pero actualmente está siendo sustituida en muchos casos por clopidogrel, ya que la ticlopidina ocasiona aparentemente un número superior de sucesos adversos hematológicos. Con estos resultados el clopidogrel puede colocarse en el grupo de cabeza del arsenal terapéutico posinfarto tras el alta hospitalaria, es decir el compuesto por los inhibidores de ECA, beta bloqueantes, estatinas y aspirina.

Introduction
Some of the presentations held at the 50th meeting of the American College of Cardiology, in Orlando, Florida, can be important in clinical Cardiology, because they are capable of conditioning the modification of current practice, or promoting certain lines of investigation in atherosclerosis and cardiovascular clinical investigation.
CURE trial
The presentation of the CURE trial (Clopidogrel in Unstable angina to prevent Recurrent Events;) has been one of the most commented ones by those present at the meeting, as well as by the media. The CURE study has its origin in CAPRIE, the previous wide-scale trial (1), in which the effects of Clopidrogel (Plavix; BMSquibb/Sanofi) were evaluated against aspirin in approximately 20,000 patients with all kinds of vascular disease (cerebro-vascular disease, coronary artery disease, peripheral vascular disease, or any combination of these). The CAPRIE study revealed a lesser risk of cardiovascular events in patients treated with clopidrogel than in patients who were given aspirin; these results were, to some extent, unexpected, because the beneficial effects of aspirin are very well documented. Also, these results posed the possibility that the concomitant administration of clopidrogel and aspirin could produce additional beneficial effects.
The CURE trial has been carried out in patients with acute myocardial infarction (MI), or unstable angina, which were randomly distributed either to a group treated with clopidrogel and aspirin, or to a second group with exclusive administration of aspirin. The participants in this study continued to receive treatment with clopidrogel after hospital discharge. It has been noticed that patients treated with both drugs achieve a decrease in the final evaluation criteria for recurrent cardiovascular events; this reduction was of approximately 20% compared to the values of patients treated exclusively with aspirin. This finding, although still preliminary and its verification waited for impatiently for its publishing, is very surprising and it will likely be soon incorporated to clinical practice. The extent of this is such that currently a vascular endo-prosthetic device (stent) is placed in many patients with acute coronary syndrome, and as a norm, after this surgical intervention, clopidrogel is administered for 1 month. Ticlodipine has also been used after the implantation of a vascular endo-prosthetic device, but it is currently being substituted in many cases for clopidrogel, because ticlodipine apparently causes a superior number of adverse hematological events. With these results, clopidrogel may be placed at the head of the therapeutic post-infarction arsenal after hospital discharge, this arsenal being the one comprised of ACE inhibitors, beta blockers, statins and aspirin.
