En la conferencia AASLD de 2002, Yeh y colegas presentaron un estudio que comparó las concentraciones de viramidina en los glóbulos rojos frente al interferón estándar en voluntarios sanos. Los estudios iniciales han mostrado que la viramidina no puede introducirse en los glóbulos rojos humanos hasta que no se ha convertido en ribavirina. Este estudio halló que una dosis de viramidina similar a la de la ribavirina estándar producía una acumulación menor en 2,4 veces de ribavirina total en los glóbulos rojos. Este resultado sugiere que la viramidina puede causar un menor grado de anemia hemolítica que la ribavirina estándar. At the 2002 AASLD Conference, Yeh and colleagues presented a study that compared viramidine concentrations in red blood cells in the presence of standard interferon in healthy volunteers. Initial studies have shown that viramidine won’t get inside red blood cells unless it has been metabolized into ribavirin. This study found that a dose of viramidine similar to that of standard ribavirin, produced a ribavirin accumulation 2.4 times smaller in RBCs. This result suggests that viramidine can cause a lesser degree of hemolytic anemia than standard ribavirin. Viramidine would potentially allow greater concentrations of ribavirin in plasma with less side effects, and this would lead to greater rates of sustained response and a better compliance by patients.
In other study presented also at the 2002 AASLD Conference, Lau and partners, described the safety and pharmacokinetic profiles of viramidine in hepatitis C patients. These investigators determined that a dose up to 800 mg of viramidine twice a day during two weeks is safe and well tolerated. No significant changes were observed in the levels of hemoglobin in the viramidine group.
Data presented at the 2002 AASLD Conference confirm that the ribavirin-derived viramidine has a much smaller likelihood to cause hemolytic anemia relative to standard ribavirin. This smaller likelihood is due to a lesser interaction between viramidine and RBCs. Pharmacokinetic effects regarding the intake of viramidine with or without meals don’t seem to be of clinical relevance. To date, it seems that viramidine has all the advantages of ribavirin but with a much more desirable side-effect profile. |
