Exploring the bowel
When we eat, food passes down the oesophagus or ‘food pipe’, into the stomach, and then into the small bowel. Most of the nutrients from our food are absorbed into our bodies in the small bowel. The large bowel, or colon, is a tube approximately 1.5 metres in length that connects the small bowel to the rectum and anus. It starts at the appendix and caecum on the lower right-hand-side of the abdomen, passes upwards to the underside of the ribs, and then across to the left-hand-side of the body and down into the pelvis. The colon is approximately 3 centimetres wide and has many important roles, including storage and excretion of the body’s waste.
A range of problems can occur in the colon, including polyps and cancer. Polyps are common but are usually benign or non-malignant, which means that that they do not spread to other parts of the body. Usually they cause no symptoms; however, it is important to identify them because they may become malignant as they enlarge, although this process can take several years. Although the great majority of polyps will never become malignant, it is believed that the majority of bowel cancers start off as small benign polyps. Bowel cancer is one of the most common malignancies, usually occurring in middle-aged or elderly people.

Why is this study being carried out?
Vitamin D was used to treat TB before antibiotics were discovered, and laboratory research shows that vitamin D can boost the immune system. We suspect that giving vitamin D in addition to antibiotics might help patients like you to recover more quickly from TB. This study is designed to find out whether this is true.
Why have I been chosen?
You have been chosen because you have been diagnosed with lung TB. Altogether, we will recruit 146 patients like you for this study.
Will I miss out on any treatment if I take part in the study?
No. Every patient in the study will receive full antibiotic treatment for lung TB. Our study is designed to look at the effect of giving vitamin D in addition to antibiotic treatment.

Although PSA remains the only serological marker available to assess the response to therapy in prostate cancer patients, its value, and the appropriate use of PSA screening remains controversial. Several factors may lower the sensitivity and specificity of PSA screening including age, race, and nonbenign prostatic diseases such as prostatitis or BPH. Recent reports suggest that genetic variation in the promoter region of the PSA gene may also contribute to the individual variations observed in serum PSA levels. In fact, Xue et al. reported that men without prostate disease, in whom the AA genotype at the -158G/A single nucleotide polymorphism (SNP) sequence was present, had higher serum PSA levels compared with AG or GG genotypes [14]. Utilizing polymerase chain reaction to amplify the promoter region of the PSA genes, Cramer and colleagues evaluated 409 healthy men to further characterize PSA gene polymorphisms and their possible associations with serum PSA levels [15]. The median age in this cohort was 63.7 (±9.1) years with a median serum PSA concentration of 1.01 ng/mL (range 0.14–20.7), 36 patients had a serum PSA >2.5 ng/mL.
In addition, two previously unreported sequence variants were identified in the androgen-responsive upstream enhancer region; a SNP and a poly(C) repeat. Neither of these variants affected serum PSA levels or PSA promoter function. In contrast, after sequencing further upstream of the PSA promoter, two additional SNPs (-5429T/G and -5412 T/C) that did affect serum PSA levels and PSA promoter activity were identified.
