INSTRUCTIONS FOR COMPLETING THE DIARY
During the trial we kindly ask you to fill in this diary on your own. Your trial doctor or nurse will advise you on how to fill it in. Please note that if you normally keep a diabetes diary you can keep the two diaries at the same time.
For three days during the last week before a clinic visit or a telephone contact you need to record the following:
• Blood glucose measurements done before breakfast, lunch, dinner and at bedtime. Please record your blood glucose readings from the meter provided. Any values below 56 mg/dL should be recorded on the ‘Hypoglycaemic Episode” tables, even if you do not have any symptoms
• All insulin doses taken on the same three days as the blood glucose measurements
During the entire trial, please record the following information. Refer to the relevant page for examples:
• Whether or not you woke up during the night due to asthma symptoms. Fill in the “Night-time Awakenings due to Asthma Symptoms” table every morning
• The number of inhalations of asthma rescue bronchodilator due to asthma symptoms you have taken during the last 24 hours. Fill in the table “Asthma Rescue Bronchodilator due to Asthma Symptoms” every morning
• Your current daily maintenance asthma medication in the “Daily Maintenance Asthma Medication” table
• Your current basal insulin and oral anti-diabetic (OADs) medication and any new medications in the ‘Illness and Medication’ table
• If you experience any hypoglycaemic episodes, please fill in the ‘Hypoglycaemic Episodes” table

ஒரு மருத்துவ வருகை அல்லது தொலைபேசித் தொடர்புக்குமுன் கடைசி வாரத்தில் மூன்று நாட்களுக்கு நீங்கள் கீழ்க்கண்டவற்றை பதிவு செய்வது அவசியமாகும்:

• அதே மூன்று நாட்களில் எடுத்துக்கொண்ட அனைத்து இன்சுலின் அளவுகளை இரத்த குளுக்கோஸ் அளவீடுகளாக எடுத்துக்கொள்ளவும்.
சோதனை முழுவதும் , தயவுசெய்து கீழ்க்கண்ட தகவல்களை பதிவு செய்யவும். உதாரணங்களுக்கு சம்பந்தப்பட்ட பக்கத்தை பார்க்கவும்:
• நீங்கள் ஆஸ்துமா அறிகுறிகளால் இரவில் தூக்கத்தில் இருந்து விழிதெழுந்தீர்களா இல்லையா. ஒவ்வொரு காலையிலும் “ஆஸ்துமா அறிகுறிகளால் ஏற்படும் இரவுநேர விழித்தெழுதல்களை” அட்டவணையில் பூர்த்தி செய்யவும்.
• கடந்த 24 மணி நேரத்தில் ஆஸ்துமா அறிகுறிகள் காரணமாக காப்பாற்றும் நுரையீரல் விரிப்பான் மூலம் எடுக்கப்பட்ட உள்ளிழுப்புகளின் எண்ணிக்கை . ஒவ்வொரு காலையிலும் “ஆஸ்துமா அறிகுறிகள் காரணமாக பயன்படுத்திய காப்பாற்றும் நுரையீரல் விரிப்பான் ” அட்டவணையைப் பூர்த்தி செய்யவும்.
• உங்கள் தற்போதைய தினசரி பராமரிப்பு ஆஸ்துமா மருந்தளவை “தினசரி ஆஸ்துமா பராமரிப்பு மருந்து “ அட்டவணையில் குறிக்கவும்.
• உங்கள் தற்போதைய கீழ்ப்புறம் கொடுக்கப்படும் இன்சுலின் மற்றும் வாய்வழி கொடுக்கப்படும் நீரழிவு எதிர்ப்பு மருந்துகள் (ஓஎடி) மற்றும் எந்த புது மருந்துகள் ஆகியவற்றை “நோய் மற்றும் மருந்துகள்” அட்டவணையில் குறிக்கவும்.
• ஹைப்போகிளைசீமிக் (குறைந்த இரத்த சர்க்கரை அளவு) நிகழ்வுகளை நீங்கள் அனுபவித்தால் தயவு செய்து அதை ‘ஹைப்போகிளைசீமிக் அட்டவணையில்’ குறிக்கவும்
