INDICAZIONI TERAPEUTICHE
Nel bambino: criptorchidismo, ipogonadismo, eunucoidismo ipogonadotropico.
Nella donna: amenorrea primaria e secondaria, ipoplasia ovarica, menometrorragia, aborto ricorrente, minaccia d'aborto, infertilità anovulatoria, sterilità da deficiente ovogenesi.
Nell'uomo: azoospermia, oligoastenospermia, astenospermia.
CONTROINDICAZIONI
Pubertà precoce. Ipertrofia o neoplasie ipofisarie, neoplasie ovariche, carcinoma prostatico o altra neoplasia androgeno-dipendente, neoplasie dei testicoli. In presenza di altre patologie endocrine (es.: ipotiroidismo, insufficienza corticosurrenalica, iperprolattinemia) deve essere prima istituita adeguata terapia. Insufficienza ovarica o testicolare primitiva, assenza dell’utero, menopausa precoce, occlusione tubarica (a meno che la paziente non sia sottoposta a programmi di fertilizzazione in vitro). Tromboflebite in fase attiva. Ipersensibilità nota al principio attivo o ad uno qualsiasi degli eccipienti.
PRECAUZIONI PER L’USO
xxxx deve essere iniettato per via intramuscolare o sottocutanea. La soluzione ottenuta unendo il solvente al liofilizzato deve essere utilizzata immediatamente dopo la preparazione.
INTERAZIONI.
Evitare l’uso contemporaneo di gonadotropina corionica con alte dosi di corticosteroidi.
AVVERTENZE SPECIALI
In caso di comparsa di segni di pubertà precoce dovuta all'induzione di secrezione androgena promossa dalla gonadotropina corionica, è necessario sospendere la terapia. Poiché, infatti, gli androgeni possono provocare ritenzione di fluidi, la gonadotropina corionica deve essere usata con cautela in pazienti con epilessie, asma o malattie cardiache o renali.
L'uso congiunto della gonadotropina corionica (HCG) con la gonadotropina umana della menopausa (HMG) dovrebbe presupporre la completa conoscenza delle controindicazioni, delle precauzioni di impiego e delle reazioni secondarie possibili, le principali delle quali sono:
ingrossamento ovarico, ascite con o senza dolore, reazioni pleuriche, rottura di cisti ovariche con conseguente emoperitoneo, nascite multiple, tromboembolie arteriose.
Pur non essendo stato riportato alcun caso di contaminazione virale associato alla somministrazione di gonadotropine estratte da urine umane, il rischio di trasmissione di agenti patogeni conosciuti o sconosciuti non può essere totalmente escluso.

ТЕРАПЕВТИЧНИ ПОКАЗАНИЯ
При момчета: хрипторхизъм, хипогонадизъм, хипогонадотропен евнухоидизъм.
При жени: първична и вторична аменорея, хипоплазия на яйчниците, менометрорагия, чести аборти, заплашваш аборт, ануволаторно безплодие, стерилитет поради недостатъчна овогенеза.
При мъже: азооспермия, олигоастеноспермия, астеноспермия.
ПРОТИВОПОКАЗАНИЯ
Ранен пубертет, хипертрофия или тумори на хипофизната жлеза, овариални тумори, карцином на простата или друга андрогенно зависима неоплазия, тумори на тестисите. При наличие на други ендокринни патологии (напр. хипотиреоидизъм, недостатъчност на кората на надбъбреците, хиперпролактинемия) първо трябва да се установи подходящата терапия. Овариална или първична тестикуларна недостатъчност, липса на матка, ранна менопауза, запушване на тръбите (ако пациентката не е включена в програми за оплождане ин витро). Тромбофлебит в активна фаза. Установена свръхчувствителност към активното вещество или към някое от помощните вещества.
ПРЕДПАЗНИ МЕРКИ ПРИ УПОТРЕБА
xxxxx трябва да се инжектира интрамускулно или подкожно. Инжекционният разтвор, получен при смесването на разредителя и лиофилизирания прах се инжектира веднага след приготвянето му.
ЛЕКАРСТВЕНИ И ДРУГИ ФОРМИ НА ВЗАИМОДЕЙСТВИЯ
Да се избягва едновременното прилагане на хорионгонадотропин с високи дози на кортикостероиди.
СПЕЦИАЛНИ ПРЕДУПРЕЖДЕНИЯ
При появата на признаци на ранен пубертет, дължащи се на индуцирането на андрогенна секреция след прилагане на хорионгонадотропин, терапията трябва веднага да се преустанови. Тъй като андрогените могат да предизвикат задържане на вода, хорионгоданодотропинът трябва внимателно да се прилага при пациенти с епилепсия, астма или сърдечни или бъбречни заболявания.
Комбинираното или последователно прилагане на хорионгонадотропин с HMG (човешки менопаузален гонадотропин) трябва да се провежда внимателно предвид вероятността от вторични реакции като увеличение на яйчниците, асцит, плеврални реакции, тромбоемболия, руптура на кисти на яйчниците, многоплодна бременност.
При спортисти: употребата на лекарствения продукт без терапевтична необходимост представлява допинг и може да даде положителен резултат при антидопингов тест.
Макар да няма данни за вирусно заразяване при прием на гонадотропини, екстрахирани от човешка урина, рискът от предаване на известни или неизвестни патогенни агенти не трябва напълно да се изключва.
