VOLUNTARY PARTICIPATION
Your participation in this study is voluntary. You cannot be forced to be in this study. You may refuse to take part in this study. You may stop your participation at any time during the study. You will not be penalized or lose benefits to which you are otherwise permitted.
If you refuse to sign this informed consent form, you will not be able to take part in this study. Even after signing this consent form, you are free to withdraw your consent and stop participation at any time. This will not alter the routine medical care you will receive from your doctor. However, for your safety, you should come back to the clinic for a final examination if you decide to stop taking part in the study.
If the symptoms of your high blood phosphorus become too severe, you will be taken out of the study and put back on your previous medication or another form of treatment recommended by your doctor. Also, if you are not able to follow the study plan, take a medication that is not allowed in the study, or experience a study-related injury or troublesome side effect, you may be taken out of the study.
In addition, the investigator, the Sponsor, the FDA or IntegReview (the IRB) can stop your participation at any time, for any reason, without your consent.
END OF STUDY INFORMATION
When you stop taking part in the study, for any reason, you will not be able to continue taking the study drug.
The study doctor will talk to you about any medical problems that may happen if you stop taking part in the study.
SUBJECT’S BILL OF RIGHTS
As a subject in a research study, you have the following rights:
1. Be told about the nature and purpose of the study.
2. Be told about the procedures used in the study and any drug or device to be used.
3. Be told about any side effects, discomforts or risks that we expect from being in this study.
4. Be told about any benefits that you can expect from this study.
5. Be told about any other procedures, drugs, or devices that might be helpful to you, and their risks and benefits.
6. Be told about any medical treatments available to you after the study if problems occur.
7. Be given a chance to ask any questions about the study or procedures.

Además, el investigador, el Patrocinador, la FDA o IntegReview (la IRB) pueden suspender su participación en cualquier momento, por cualquier motivo, sin su consentimiento.
INFORMACIÓN FINAL DEL ESTUDIO
Cuando suspenda su participación en este estudio, por cualquier motivo, no podrá continuar tomando la droga en estudio.
El médico del estudio hablará con usted acerca de cualquier problema médico que pueda sucederle si suspende su participación en el estudio.
