Zaburzenia układu sercowo – naczyniowego i zespół zmęczenia u chorych na stwardnienie rozsiane.
Mimo, że stwardnienie rozsiane jest chorobą dotyczącą głównie centralnego układu nerwowego, opisywane są także przypadki zajęcia układu obwodowego i autonomicznego. Powszechnie znanymi i często spotykanymi objawami uszkodzenia tych układów są zaburzenia seksualne, zaburzenia zwieraczy czy zespół zmęczenia. Opisywane są także przypadki dysfunkcji układu sercowo- naczyniowego. Dokładnie zebrany wywiad i badanie przedmiotowe wskazuje jednak, że nie są to przypadki sporadyczne. W celu dokładniejszego zbadania tego problemu grupa niemieckich naukowców przeprowadziła badanie, w którym oceniała częstość i rodzaj występowania zaburzeń układu autonomicznego u chorych na stwardnienie rozsiane.
W badaniu wzięło udział 84 pacjentów chorych na SM, w tym 54 z typem klinicznym przebiegającym z rzutami i remisjami, 14 z postacią wtórnie postępującą i 16 z postacią pierwotnie postępującą. W celu oceny sprawności układu autonomicznego, u każdego chorego przeprowadzono różne testy diagnostyczne, w tym ocenę tętna i ciśnienia tętniczego przy próbie pionizacji, próbę Valsalwy, zmianę częstości rytmu pracy serca pod wpływem hiperwentylacji i zmiany ciśnienia krwi pod wpływem zaciskania dłoni. Wyniki uzyskane w każdym z testów zostały następnie przeliczone na ogólna skalę dysfunkcji układu autonomicznego. Wyróżniono w niej trzy stopnie zaburzeń: poważne, umiarkowane i słabe. Dodatkowo, na podstawie trzech odpowiednio przygotowanych ankiet oceniono stopień uczucia zmęczenia występującego u chorych.
Analiza uzyskanych wyników wykazała, że biorąc pod uwagę wyniki uzyskane we wszystkich testach ponad 27% pacjentów miało cechy uszkodzenia układu autonomicznego. Najwięcej nieprawidłowych wyników uzyskano u chorych z pierwotnie postępująca postacią SM, nieco mniej z postacią wtórnie postępującą. Najmniej cech uszkodzenia tego układu stwierdzono u pacjentów z typem klinicznym przebiegającym z rzutami i remisjami. We wszystkich badanych podgrupach chorych podobnie często uszkodzony był układ sympatyczny jak i parasympatyczny. U ponad 25% pacjentów stwierdzono cechy ortostatycznego spadku ciśnienia przy próbie pionizacji, a u trzech wystąpiło nawet omdlenie. Podobnie często występowała zmiana rytmu pracy serca pod wpływem hiperwentylacji. Ten parametr korelował także ze stopniem deficytów neurologicznych ocenianych w skali EDSS. Nie stwierdzono natomiast zależności pomiędzy stanem neurologiczny pacjentów a całkowitym stopniem zaburzeń autonomicznych. Ocena występowania uczucia zmęczenia wykazała, że aż ponad 64% pacjentów skarżyło się na takie objawy. Uczucie zmęczenia częściej występowało u mężczyzn niż u kobiet i nie miało związku, ani z czasem trwania choroby, ani ze stopniem deficytów neurologicznych. Ciekawych informacji dostarczyła analiza wyników testów autonomicznych w odniesieniu do występowania uczucia zmęczenia. Ponad 19 % pacjentów z zaburzeniami układu autonomicznego skarżyło się także na uczucie zmęczenia. Pacjenci ze znacznym nasileniem zmęczenia charakteryzowali się najczęściej nieprawidłowymi wynikami próby Valsalvy i zmianami rytmu pracy serca pod wpływem hiperwentylacji a rzadko zmianami ciśnienia tętniczego przy pionizacji.
Wyniki uzyskane w tym badaniu jednoznacznie potwierdzają wcześniejsze przypuszczenia o uszkodzeniu układu autonomicznego w przebiegu stwardnienia rozsianego. Ze względu na charakter objawów klinicznych spowodowanych takim uszkodzeniem wydaje się być istotnym, aby przy opracowywaniu kolejnych strategii leczniczych SM, brano pod uwagę także ten problem.

Disorders of cardiovascular system and fatigue syndrome in multiple sclerosis patients
Despite the fact that multiple sclerosis is a disease which attacks mainly the central nervous system, cases have been reported in which the peripheral and autonomic nervous systems were also affected. Commonly known and frequently encountered symptoms occurring when the latter two are afflicted include sexual disorders, sphincter disorders or fatigue syndrome. Cases of cardiovascular system dysfunction have also been recorded. Detailed medical history of the affected patients and medical examination reveal, however, that those cases are not isolated. To examine the issue further, a group of German scientists conducted a research on the frequency and disorder types of the autonomic nervous system encountered by the multiple sclerosis sufferers.
The research was carried out on 84 patients with different clinical types of multiple sclerosis, including 54 cases involving relapsing and remitting, 14 cases of secondary-progressive MS and 16 cases of the primary-progressive type. In order to examine the condition of the autonomic nervous system, each patient underwent a number of diagnostic tests including, among others, pulse and arterial pressure measurements upon the assumption of an upright position, the Valsalva manoeuvre, alterations in the heart rate induced by hyperventilation and changes in the blood pressure provoked by fist clenching. The results achieved in each of the tests were then, calculated to form a scale of autonomic nervous system dysfunction. The scale distinguishes three degrees of disorders: serious, moderate and weak. Additionally, the degree of fatigue felt by the patients was scored based on three specially designed questionnaires.
The analysis of the findings revealed that 27% of the patients who underwent the tests displayed the damage of the autonomic nervous system. The highest score of negative results was observed in the patients with primary-progressive MS, then respectively, in the patients with secondary-progressive MS and, finally, in those with relapsing-remitting MS. All tested subgroups demonstrated an equal degree of damage to the sympathetic and parasympathetic nervous systems. 25% of the patients displayed orthostatic hypotension upon the assumption of an upright position and even, three cases of fainting were reported. The alterations in the heart rate induced by hyperventilation were equally, often recorded. This parameter also revealed a correlation with the degree of the neurological deficits scored on the EDSS scale. The relationship between the patients’ neurological condition and the overall degree of autonomic system disorders was not attested. The analysis on the occurrence of fatigue showed that over 64% were affected by its symptoms. The fatigue was reported to affect more often men than women and to display relation neither with the duration of the disease nor with the degree of neurological deficits. The analysis of autonomic test results provided some interesting information with reference to the occurrence of fatigue. Over 19% of the patients with autonomic system disorders were reported to exhibit fatigue symptoms. The patients who demonstrated a higher degree of fatigue most often were reported to have negative results in the Valsalva manoeuvre and to display alterations in the heart rate induced by hyperventilation, however, rarely had they encountered alterations in arterial pressure upon the assumption of an upright position.
The results obtained in this research explicitly confirm previous predictions about the damaging effect of multiple sclerosis on the autonomic nervous system. On the account of nature of clinical symptoms caused by the damage of this type, it appears that this problem needs to be taken into consideration while designing new treatment strategies for MS.
