Introduction
Hepatitis C virus (HCV) infection is a major cause of chronic liver disease and affects an estimated 180 million people worldwide.[1-3] Approximately 70% of acute HCV results in chronic infection of which more than 20% will develop liver cirrhosis. Further disease progression to hepatocellular carcinoma is observed in 5% of HCV-infected patients with cirrhosis.[4] The most common means of HCV transmission is through parenteral exposure.[5]
The current standard for antiviral treatment consists of combination therapy with peginterferon alfa and ribavirin for 24 or 48 weeks depending on viral genotype and is associated with a sustained virologic response (SVR) in approximately 46% to 80% of patients.[6,7] Although response rates are favourable, a number of studies indicate that HCV therapy is associated with several adverse effects of varying severity in many patients and can lead to cytokine-induced depression in approximately 20% to 40% of treated patients.[8-10] Finally, adverse events associated with HCV therapy, such as depression, can lead to noncompliance or even premature discontinuation.[11-14] This finding has led some experts to suggest that interferons not be given to HCV-infected patients who have preexisting psychiatric symptoms.[15]
