Existem duas situações básicas para a realização de exames de neuroimagem em pacientes com diagnóstico de epilepsia. A primeira se aplica a paciente recém diagnosticada e àqueles com epilepsia de longa data que ainda não foram devidamente investigados. A segunda se aplica a pacientes com epilepsia de difícil controle e, portanto, candidatos a um tratamento cirúrgico (Commission on Neuroimaging of the ILAE. 1997). Mesmo pacientes com epilepsia parcial de longa duração sem etiologia definida devem ser submetidos a um exame de neuroimagem. Oligodendrogliomas ou outros tumores de baixo grau têm sido descritos em pacientes com história de epilepsia com mais de 20 anos de duração.
A prioridade deve ser dada a pacientes com alterações focais no exame neurológico. Exames de urgência (tomografia ou RM) devem ser realizados em pacientes que apresentam as primeiras crises com o aparecimento de déficits neurológicos focais, febre, cefaléia persistente, alterações cognitivas e história recente de trauma craniano. Crises parciais com início após os 40 anos de idade devem ser consideradas como possível indicação para um exame de emergência.

Existen dos situaciones básicas para la realización de estudios por neuroimagen en pacientes con diagnóstico de epilepsia. La primera, se aplica al paciente recién diagnosticado y al paciente con epilepsia de larga data que aún no ha sido debidamente investigado. La segunda, se aplica a pacientes con epilepsia de difícil control y, por lo tanto, candidatos al tratamiento quirúrgico (Commission on Neuroimaging of the ILAE, 1997). Aun aquellos pacientes con epilepsia parcial de curso prolongado sin etiología definida deben someterse a un estudio por neuroimagen. Oligodendrogliomas u otros tumores de bajo grado han sido descritos en pacientes con historia de epilepsia de más de 20 años de duración.
Se deben priorizar aquellos pacientes que presentan alteraciones focales al examen neurológico. Deben realizarse estudios de urgencia (tomografía o RM) en aquellos pacientes que presentan las primeras crisis con la aparición de déficit neurológicos focales, fiebre, cefalea persistente, alteraciones cognitivas e historia reciente de trauma craniano. Las crisis parciales con inicio después de los 40 años de edad deben tomarse en cuenta como posible indicación para un examen de urgencia.
