This study is a Phase II-A, multicenter, randomized, double-blind, placebo-controlled, proof of concept study. The study consists of two parts. Part 1 is a dose ascension study to evaluate the safety and tolerability of escalating doses of XXX-000000 administered as an IV. Part 2 will be a dose confirmation study where the highest tolerated dose determined in Part 1 will be used to test whether there will be an improvement in angiographic situation and ST-segment resolution with IV XXX-000000.
Subjects with acute STEMI who are planned to undergo primary PCI will be randomized to receive an IV of XXX-000000 or placebo prior to the intervention. In Part 1, the first dose group (10 mg) will evaluate a total of 10 subjects and then the following dose groups (20 mg, 40 mg, 60 mg) will evaluate 20 subjects each. All subjects in Part 1 will be randomized in a 1:1 manner to either XXX-000000 or placebo. In Part 2, a total of 150 subjects will be randomized 1:1 to either XXX-000000 or placebo.
Both parts will involve:
▪ STEMI subjects who are to undergo primary PCI and who receive additional standard medical therapy (acetylsalicylic acid, heparin, and/or clopidogrel). GP IIb-IIIa inhibitor use is allowed and encouraged after the initial angiogram has been obtained.
▪ The timing of the initial clopidogrel loading dose (300 or 600 mg) is at the discretion of the investigator, however, a repeat 300 mg loading dose is recommended for all subjects 6 hours after PCI given potential competitive interactions between XXX-000000 at high concentration and clopidogrel for the ADP receptor. | Translation - German
Diese Studie ist eine multizentrische, randomisierte, doppelblinde, placebokontrollierte Machbarkeitsstudie der Phase IIa. Die Studie besteht aus zwei Teilen. Der 1. Studienteil umfasst eine Dosissteigerung zur Beurteilung der Sicherheit und Verträglichkeit von intravenös verabreichtem XXX-000000 mit steigender Dosierung. Der 2. Studienteil umfasst die Bestätigung mit der in Teil 1 festgelegten maximal verträglichen Dosis, um zu bestimmen ob ein Zusammenhang zwischen einer verbesserten angiografischen Gefäßsituation sowie Rückbildung der ST-Hebungen und intravenösem XXX-000000 nachgewiesen werden kann.

Beide Studienteile umfassen:
▪ Patienten mit ST-Hebungsinfarkt, bei denen eine primäre perkutane Koronarintervention vorgesehen ist und die außerdem zusätzliche medizinische Standardbehandlungen (Acetylsalicylsäure, Heparin und/oder Clopidogrel) erhalten.
