In 2001, 2 studies suggested that survival of cancer patient could be improved by EPO therapy. Head
and neck cancer patient undergoing radiotherapy had worse survival = less chance of survival or worse survival rate if they were anaemic, but, compared to historical controls, EPO therapy restored survival to that of non-anaemic patients (Glaser, 2001).
A placebo-controlled study in cancer patient on chemotherapy observed a non-significant trend of extended survival in patients receiving EPO therapy (Littlewood, 2001). A first meta-analysis carried out by the Cochrane group on 21 randomized trials confirmed that patients receiving EPO therapy had a significantly better survival than controls (Hazard ratio, HR 0.81 [0.67 to 0.99]) (Bohlius, 2005). This prompted the launch of prospective randomized trials testing the hypothesis that EPO therapy could improve survival of cancer patients. However, the first 2 published trials came out with the opposite outcome. A first trial in 351 patients with head and neck cancer undergoing curative radiotherapy and randomized to placebo or EPO therapy to target an Hb > 14 g/dL in females or > 15g/dL in males resulted in worse locoregional progression-free survival (PFS) (Henke, 2003).

En 2001, deux études ont suggéré que le traitement par EPO peut améliorer la survie des patients atteints du cancer. Les patients souffrant d’un cancer de la tête et du cou qui suivent une radiothérapie avaient une moins bonne survie = moins de chance de survie ou une diminution du taux de survie s’ils étaient anémiques, mais par rapport aux contrôles historiques, le traitement par EPO ramenait ce taux au même niveau que celui des patients non anémiques (Glaser, 2001).
Un essai contrôlé contre placebo portant sur des patients atteints d’un cancer sous chimiothérapie, révèle une tendance non significative, à une survie prolongée chez les patients suivant un traitement par EPO (Littlewood, 2001). Une première méta-analyse de 21 essais aléatoires menée par le groupe Cochrane, a confirmé que la survie des patients suivant un traitement par EPO était significativement meilleure que les témoins (Risque relatif, RR 0,81 [0,67 à 0,99]) (Bohlius, 2005).
Cela a incité à réaliser des essais prospectifs randomisés destinés à tester l’hypothèse selon laquelle le traitement par EPO pourrait améliorer la survie des patients atteints du cancer. Cependant, les deux premiers essais publiés présentaient le résultat inverse. Un premier essai réalisé sur un échantillon de 351 patients souffrant d’un cancer de la tête et du cou, suivant une radiothérapie à visée curative et soumis de façon aléatoire à un traitement placebo ou par EPO, pour atteindre un taux d’hémoglobine (Hb) cible > 14 g/dl chez les femmes ou > 15 g/dl chez les hommes, a démontré une baisse de la survie sans progression locorégionale (SSP) (Henke, 2003).
