Cannabis et grossesse
Le cannabis est le toxique illicite le plus fréquemment
consommé par les femmes enceintes : au moins 3 % des
grossesses seraient concernées. Plusieurs études se sont
intéressées aux conséquences de son usage au cours de la
grossesse mais leurs résultats sont discutables du fait de
la présence de facteurs confondants tels qu’une polyconsommation
de substances psychoactives, licites ou illicites,
de conditions de vie parfois précaires ou de l’existence
de comorbidités psychiatriques altérant les liens maternels
précoces. De plus, l’évaluation de risque doit prendre en
compte l’exposition aux cannabinoïdes (principalement le
tétrahydrocannabinol [THC]) mais aussi aux substances liées
au mode de consommation (principalement inhalation des
fumées de cigarette élaborée après son mélange au tabac).
La présence de récepteurs cannabinoïdes au niveau placentaire
et la liposolubilité du THC lui permettent de
traverser le placenta. Après administration par voie inhalée
ou IV, les concentrations foetales en THC atteignent 30 %
des valeurs maternelles (données animales). Dans l’espèce
humaine, les cannabinoïdes sont présents dans le sang du
cordon ombilical, avec des ratios plasma maternel/sang du
cordon compris entre 2,7 à 6 pour le THC et 1,7 à 7,8 pour
son métabolite. Par ailleurs, le métabolite du THC peut être
retrouvé dans le méconium des nouveau-nés.
Les résultats contradictoires des études animales et leur
absence de reproductibilité ne sont pas en faveur d’un
potentiel tératogène des cannabinoïdes. Chez l’homme, les
données de tératogenèse et de carcinogenèse sont également
controversées. Des publications isolées décrivent des
atteintes variées n’ayant pas été confirmées (défaut du septum
ventriculaire ; survenue de leucémies lymphoblastiques
aiguës ou de neuroblastomes chez les enfants exposés in
utero) ou ne résistant pas à la prise en compte d’autres facteurs
de risque connus (par exemple, gastroschisis et jeune âge maternel). Le cannabis n’est pas associé à un tableau
malformatif typique et n’est pas reconnu comme un agent
tératogène dans l’espèce humaine.
L’abus ou la dépendance au cannabis pourrait entraîner
une diminution de la perfusion utéroplacentaire liée à une
tachycardie et des variations tensionnelles chez la mère. Les
conséquences pour l’embryon sont dominées par un retard
de croissance intra-utérien et d’accouchements prématurés.
Ce risque est corrélé à l’intensité de la consommation.
Ainsi, une consommation régulière et importante (plus de
six joints par semaine) serait associée à une réduction de
la durée de la grossesse (de 0,8 semaine en moyenne) et un
retard de croissance intra-utérin avec diminution du poids
(de 80 à 105 g), de la taille et du périmètre crânien par rapport
à l’âge gestationnel. Une consommation occasionnelle
n’entraînerait qu’un décalage de l’évolution pondérale sans
réel retard de croissance. Ces complications, semblables à
celles du tabagisme, ne permettent pas de conclure quant à
la responsabilité intrinsèque des cannabinoïdes. Toutefois,
la combustion du cannabis majorerait la carboxyhémoglobinémie
induite par la consommation de tabac (environ cinq
fois plus élevée).
Des manifestations néonatales peuvent être observées
après exposition prolongée in utero (tremblements, sursauts
exagérés, diminution de la réaction aux stimuli
—notamment visuels— et troubles de la succion, par
exemple). Constatées entre 60 et 80 heures après la naissance,
elles régressent généralement dans les semaines qui
suivent la naissance.
Le cannabis pourrait, en outre, être un facteur de
risque de mort subite du nourrisson. En 2001, une étude
cas—témoins a montré une augmentation de ce risque
(OR = 2,23 avec IC 95 % entre 1,39 et 3,57) chez les nourrissons
de mère ayant consommé du cannabis au cours de leur
grossesse et cela, indépendamment des caractéristiques
ethniques et de la consommation de tabac.
En ce qui concerne les effets à long terme, plusieurs
études épidémiologiques ont relevé des anomalies
comportementales et cognitives chez les enfants nés de
femmes ayant consommé régulièrement du cannabis durant
leur grossesse, parmi lesquelles une augmentation de l’impulsivité et de l’hyperactivité, des déficits de l’attention
ou des fonctions exécutives (capacité de planification ou
de jugement. . .), des troubles de l’apprentissage (lecture,
compréhension et performances scolaires) ou des troubles
des conduites (risque d’initiation de consommation de tabac
et de cannabis à l’adolescence), sans diminution globale du
QI. Le mécanisme en cause est encore incomplètement élucidé
et les facteurs socioculturels pourraient avoir un rôle
prépondérant. Cependant, on doit évoquer l’implication des
endocannabinoïdes dans la maturation cérébrale, bien mise
en évidence chez le foetus animal exposé. L’apport exogène
de THC pourrait perturber cette fonction pendant la
grossesse.
En conclusion, aucune conséquence délétère majeure n’a
pu être formellement établie en ce qui concerne l’exposition
aux cannabinoïdes au cours de la grossesse. Cependant, les
incertitudes quant aux répercussions neurocomportementales
chez l’enfant et son association (quasi) systématique
au tabac (clairement néfaste pour la grossesse), doivent
conduire à rechercher son usage au cours de la grossesse
afin d’inciter la mère à l’arrêt de cette consommation difficilement
avouée de par son caractère illicite.

Cannabis and pregnancy
Cannabis is the most frequently used illegal toxin in pregnant women: At least 3% of pregnancies would be affected. Several studies dealt with the consequences of its use during pregnancy, but their results are questionable because of the presence of astonishing factors such as the multiple use of psychoactive substances, licit or illicit, living conditions sometimes precarious or the existence of psychiatric co-morbidities altering early maternal links. Moreover, risk assessment must take into account exposure to cannabinoids (mainly tetrahydrocannabinol [THC]), but also substances related to the mode of consumption (mainly inhalation of cigarette smoke developed after its mixture with tobacco).
The presence of cannabinoid receptors at the placental level and the fat solubility of THC allow it to cross the placenta. After administration through the inhalation or IV, fetal THC concentrations reach 30% of maternal values (animal data). In humans, cannabinoids are present in umbilical cord blood, with maternal plasma / cord blood ratios between 2.7 to 6 for THC and 1.7 to 7.8 for its metabolite. Moreover, the THC metabolite of can be found in the newborn’s meconium.
Contradictory results of animal studies and their lack of reproducibility do not support the teratogenic potential of cannabinoids. In humans, Teratologenesis and carcinogenesis data are equally controversial. Remote publications describe various effects that have not been confirmed (ventricular septum defects; occurrence of acute lymphoblastic leukemia or neuroblastoma in children exposed in utero) or that do not resist when taking into account other known risk factors (for example, gastroschisis and early maternal age). Cannabis is not associated with a typical malformative picture and is not recognized as a teratogenic agent in humans.
Cannabis dependence or abuse could lead to a reduction utero-placental infusion linked to tachycardia and blood pressure variations in the mother. The consequences for the embryo are dominated by premature deliveries and intra-uterine growth delay. This risk is correlated to the intensity of consumption. Regular and significant consumption (more than six joints per week) would be associated with a reduction of the duration of pregnancy (from 0.8 week on average) and intrauterine growth retardation with reduction in weight (80 to 105 g), cranial size and circumference in relation to gestational age. Occasional consumption would only cause a lag in weight progression without real growth delay. These complications, similar to those of tobacco use, do not allow us to conclude as to the inherent responsibility of cannabinoids. However, the burning of cannabis increases carboxyhemoglobinemia caused by tobacco use (about five times higher).
Neonatal events can be observed after prolonged exposure in utero (tremors, exaggerated starts, decrease in response to stimuli – namely, visuals - and suction disorders, for example). Observed between 60 and 80 hours after birth, they generally regress in the weeks following birth.
Furthermore, Cannabis could be a risk factor for sudden infant death syndrome. In 2001, a case control study showed an increase in this risk (OR = 2.23 with IC 95% between 1.39 and 3.57) in infants of mothers having consumed cannabis throughout their pregnancy and this, regardless of ethnic characteristics and tobacco use.
With respect to long-term effects, several epidemiological studies have identified behavioral and cognitive anomalies in children born to women who regularly consumed cannabis during pregnancy, including an increase in impulsiveness and hyperactivity, attention or executive function deficits (planning ability or judgment, etc.), learning disabilities (reading, comprehension, and academic performance) or conduct disorders (risk of introduction to tobacco and cannabis in adolescence), with no overall decrease in I.Q. The mechanism involved is still incompletely elucidated and socio-cultural factors could play an important role. However, we should evoke endocannabinoids involvement in cerebral maturation, well highlighted in the exposed animal fetus. The exogenous contribution of THC could disrupt this function during pregnancy.
In conclusion, no major deleterious consequences could be formally established with respect to exposure to cannabinoids during pregnancy. However, the uncertainties with respect to neurobehavioral repercussions in the child and (quasi) systematic association to tobacco (clearly detrimental to the pregnancy), should lead to the search for its use during pregnancy in order to encourage the mother to stop its use, difficult to admit due to its unlawful nature.
