Lipanthyl (also marketed as Fenofibrate©) is a third-generation lipid-lowering phenoxyacetic acid derivative. Its chemical name is 1-methylethyl2-[4-(4-chlorobenzoyl) phenoxy]-2-methly-propanoate. Mechanisms of action include: 1) suppressing HMG-CoA reductase, and decreasing total cholesterol synthesis in cells; 2) increasing low-density lipoprotein (LDL) receptors, and LDL passage into hepatic cells, thus expediting TC metabolism; 3) activating lipoprotein lipase activity and increasing LDL catabolism, thus decreasing TC serum concentration. Studies have reported the following adverse effects following lipanthyl therapy: nausea, diarrhoea, abdominal pain, skin rash, measles. Other symptoms include loss of strength, head aches, dizziness, insomnia, and occasionally, elevation of serum transminase, and rarely, rhabdomyolysis. Diagnostic criteria of rhabodomyolysis are the following: 1) symptoms include loss of muscle strength, muscle pain, and lameness; 2) serum CPK levels ten times higher than normal range; 3) unremarkable inflammatory changes on muscle biopsy; 4) myopathy on electromyography; 5) myosin. Primary diagnostic criterions of rhabdomyolysis are 1) and 2). In the present case, the patient exhibited muscle pain, and significantly elevated liver function and CPK values ten times higher than those indicated in the diagnosis of rhabdomyolysis. Rhabdomyolysis and liver injury were thus diagnosed and lipanthyl therapy discontinued. The patient recovered uneventfully upon cessation and did not exhibit any renal function injuries |
