Hypertension
COZAAR and HYZAAR are indicated for the treatment of hypertension. COZAAR may be used alone or in combination with other antihypertensive agents, including diuretics. HYZAAR is not indicated for initial therapy of hypertension, except when the hypertensionis severe enough that the value of achieving prompt BP control exceeds the risk of initiating combination therapy in these patients.
Stroke Risk Reduction
COZAAR and HYZAAR are indicated to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy (LVH), but there is evidence that this benefit does not apply to black patients.
Selected Important Cautionary Information
USE IN PREGNANCY: When used in pregnancy during the second and third trimesters, drugs that act directly on the renin-angiotensin system can cause injury and even death to the developing fetus. When pregnancy is detected, COZAAR and HYZAAR should be discontinued as soon as possible. See WARNINGS, Fetal/Neonatal Morbidity and Mortality.
COZAAR and HYZAAR are contraindicated in patients who are hypersensitive to any component of these products. Because of the hydrochlorothiazide (HCTZ) component, HYZAAR is also contraindicated in patients with anuria or hypersensitivity to othersulfonamide-derived drugs.
In patients who are volume-depleted, symptomatic hypotension may occur after initiation of therapy with COZAAR. This condition should be corrected prior to administration of COZAAR, or a dosage of COZAAR 25 mg should be used. In patients with a history of hepatic impairment, a starting dose of COZAAR 25 mg should be used.
HYZAAR is not recommended for patients with severe renal impairment (creatinine clearance ≤30 mL/min). HYZAAR is not recommended as a means of titration for patients with hepatic impairment because the 25-mg starting dose of COZAAR cannot be given. In patients who are volume-depleted (eg, those treated with diuretics), symptomatic hypotension may occur after initiation of therapy with HYZAAR. This condition should be corrected prior to administration of HYZAAR.
Angioedema, which may cause airway obstruction, has been reported rarely in patients treated with losartan; some of these patients previously experienced angioedema with other drugs including ACE inhibitors. Vasculitis, including Henoch-Schönlein purpura, has been reported with losartan. Anaphylactic reactions have been reported.

Riduzione del rischio d’infarto
COZAAR e HYZAAR sono indicati per la riduzione del rischio d’infarto nei pazienti con ipertensione e ipertrofia del ventricolo sinistro (IVS), ma è stato provato che i pazienti di colore rappresentano un’eccezione a questo caso.
Precauzioni importanti
USO IN GRAVIDANZA: se assunti durante il secondo e il terzo trimestre della gravidanza, i farmaci che agiscono direttamente sul sistema renina-angiotensina possono causare complicazioni e perfino la morte del feto. Non appena constatato lo stato di gravidanza, l’assunzione di COZAAR e HYZAAR deve essere interrotta il prima possibile. Si veda AVVERTENZE, Malattia e Mortalità Fetale/Neonatale.
L’uso di COZAAR e HYZAAR è controindicato nei pazienti affetti da ipersensibilità ai componenti di questi prodotti. Inoltre, a causa della presenza di idroclorotiazide, l’uso di HYZAAR è controindicato nei pazienti con anuria o ipersensibilità a sulfamidici.
In pazienti affetti da deplezione di volume, può manifestarsi ipotensione sintomatica in seguito all’inizio dell’assunzione di COZAAR. Questa condizione va curata prima della somministrazione di COZAAR o si deve far ricorso a un dosaggio di COZAAR da 25 mg. In pazienti con insufficienza epatica in anamnesi, è consigliata un dosaggio iniziale di COZAAR da 25 mg.
L’uso di HYZAAR è sconsigliato nei pazienti con insufficienza renale grave (clearance della creatinina ≤30 ml/min). HYZAAR è sconsigliato come mezzo di titolazione nei pazienti con insufficienza epatica poiché la dose iniziale di COZAAR da 25 mg non può essere somministrata. In pazienti affetti da deplezione di volume (come coloro che assumono diuretici), può manifestarsi ipotensione sintomatica dopo l’inizio della terapia con HYZAAR. Questa condizione va curata prima della somministrazione di HYZAAR.
Casi di angioedema, che causa l’ostruzione delle vie respiratorie, sono stati riscontrati raramente in pazienti che assumevano Iosartan; alcuni di essi avevano precedentemente sofferto di angioedema assumendo altri farmaci, inclusi inibitori ACE. Si registrano casi di vasculite, Porpora di Schönlein-Henoch inclusa, in concomitanza con l’assunzione di Iosartan. Si sono verificati casi di reazione anafilattica.
