The Origins and treatment of childhood Catareact
Although cataract surgery for adults is a routine procedure around the world, the same cannot be said for pediatric cataract. Surgery in children can be complex and challenging, not simply, of course, because cataracts obscure the image received by the retina but because the retinal deprivation retards the development of the visual pathway, possibly leading to an intractable form of amblyopia.
Worldwide, cataracts are one of the most important causes of lifelong visual impairment.
Visual loss from untreated cataract is uncommon in western countries but is still problematic in some developing nations.
A Chilean Cohort
One ophthalmologist on the front lines of research into the cause of congenital cataract is J. Bronwyn Bateman, MD, a genetist and professor of ophthalmology at the university of Colorado in Denver. Over the last years, Dr. Bateman has studied more than 50 large, multigenerational families in Latin America to isolate the genetic causes of cataracts and to catalog their characteristics. Through this work she identified a new locus for autosomal dominant cataract on chromosome 19. Most recently, she has been interested in the clinical variability of cataracts among a cohort of Chilean families that manifests pediatric cataracts inherited in an autosomal dominant pattern.

على الرغم من أنّ جراحة الكاتاراكت لدى الراشدين تعتبر إجراءً روتينياً في العالم أجمع إلا أنه لا يمكن قول الأمر نفسه في جراحة الكاتاراكت لدى الأطفال التي يمكن أن تكون معقّدة ومتحدّية لأن الكاتاراكت طبعا لا يعتّم الصورة التي تتلقّاها شبكيّة العين فحسب بل لأنّ حرمان الشبكية يؤخّر امتداد المسار البصري والذي يمكن أن يؤدّي ذلك إلى حالة مستعصية من العين الكسولة.
يعتبر الكاتاراكت في العالم أجمع من أهم الأسباب المؤدّية إلى العمى لدى الأطفال ومن أكثر الأسباب التي يمكن الوقاية منها في الإعاقة البصرية لمدى الحياة. كما يعدّ سبب فقدان البصر من جرّاء عدم معالجة الكاتاراكت أمراَ غير مألوف في البلدان الغربية إلا أنّه ما زال يسبب إشكالية في بعض الدول النامية.

تعتبر الطبيبة ج. برونوين باتمان, وهي عالمة جينات وبروفيسورة في تعليم طب العيون في جامعة كولورادو في دنفر, من أهم أطباء العين وفي مقدّمة القائمين بالأبحاث حول أسباب الكاتاراكت الخلقي. فقد قامت الطبيبة باتمان بدراسة أكثر من خمسين حالة ضمن عائلات كبيرة ومتعدّدة الأجيال, على مرّ خمسة عشر سنة في أميركا اللاتينية بهدف عزل الأسباب الجينية للكاتاراكت والقيام بتصنيفها.
وقد استطاعت هذه الطبيبة من خلال هذه الدراسة, أن تتعرّف على موضع جديد للكاتاراكت في صفة صبغيّة جسديّة سائدة في الصبغي 19. وحديثا ً, بدأت تثير اهتمامها تغيّرات الكاتاراكت السريري في جماعة من العائلات الشيليّة التي تظهر فيها علامات من الكاتاراكت لدى الأطفال الموروث في شكل صفة صبغيّة جسديّة سائدة.
