RATIONALE: xxx is a histone deacetylase inhibitor (HDACi). Regulation of the acetylation of both histone and non-histone proteins by histone deacetylase enzymes is one of the key mechanisms involved in epigenetic control of gene expression. Histone deacetylase inhibitors (HDACi) have demonstrated activity in both in vitro cytotoxicity, and in vivo tumour xenograft studies (Beckers et al, 2007; Hess-Stumpp et al, 2007; Qian et al, 2006; Qian et al, 2007). In addition, HDACi have been successful in the clinic (Bolden et al, 2006; Bates and Piekarz, 2007; Santini et al, 2007).

ONDERBOUWING: xxx is een histondeacetylaseremmer (HDACi). Regulatie van de acetylering van zowel histon- als non-histoneiwitten door histondeacetylasen is een van de sleutelmechanismen die betrokken zijn bij de epigenetische controle van genexpressie. De activiteit van histondeacetylaseremmers (HDACi) is aangetoond, zowel als in-vitrocytotoxiciteit als bij in-vivo-onderzoeken met tumorxenotransplantaten (Beckers et al., 2007; Hess-Stumpp et al., 2007; Qian et al., 2006; Qian et al., 2007). Monoclonal antibodies (mAbs) are antibodies that are produced in the laboratory from a single clone of immortalised antibody-producing B cells. As a clone is derived from a single cell and individual B cells produce only one specification of antibody, the antibodies produced from a B-cell clone are all identical and bind to the same part (epitope) of the same target antigen.
Monoclonal antibodies can be used therapeutically to recognise and selectively bind to antigens (proteins) expressed on the surface of malignant cells, bringing about the death of those targeted cells. Monoclonal antibodies are most commonly used in their unconjugated (naked) form, but can also be conjugated (linked) to therapeutic toxins, chemotherapy molecules or radioisotopes, thus exploiting the targeting ability of antibodies to deliver the toxin, chemotherapy molecule or radioisotope to disseminated tumour sites. Monoclonal antibodies linked to nontherapeutic radioisotopes are also sometimes used for imaging tumours and identifying sites of malignant disease.

Monoklonale antilichamen (mAb’s) zijn antilichamen die onder laboratoriumomstandigheden geproduceerd worden uit een enkele kloon van onsterfelijk gemaakte antilichaamproducerende B-cellen. Doordat een kloon afkomstig is van één enkele cel en individuele B-cellen slechts één specifiek antilichaam aanmaken, zijn de antilichamen die geproduceerd worden door een B-celkloon alle identiek en binden deze zich aan hetzelfde gedeelte (epitoop) van hetzelfde doelwitantigeen.
Monoklonale antilichamen kunnen therapeutisch toegepast worden om antigenen (eiwitten) die op het oppervlak van maligne cellen tot expressie gebracht worden, te herkennen en zich selectief eraan te binden, waardoor deze doelwitcellen gedood kunnen worden. Monoklonale antilichamen worden meestal toegepast in een ongeconjugeerde (pure) vorm, maar kunnen ook geconjugeerd (gekoppeld) worden met therapeutische toxinen, chemotherapeutische moleculen of radio-isotopen. Op deze manier wordt de doelgerichte werking van antilichamen gebruikt om het toxine, het chemotherapeuticum of het radio-isotoop aan gemetastaseerde tumoren af te geven. Monoklonale antilichamen die gekoppeld zijn aan non-therapeutische radio-isotopen worden soms ook gebruikt voor beeldvorming en opsporing van tumoren in het lichaam.

In dem Cochrane-Review zur Misteltherapie (2008) wird resümiert, dass die vorhandene Evidenz nicht ausreiche, um einen zwingenden Einsatz der Misteltherapie zu unterstützen. Dieses Resümee ist sicherlich konsensfähig. Die weitere Beurteilung jedoch, dass die Studienevidenz generell schwach sei, bedarf kritischer Sichtung.
Allgemein ist zu sagen: Das Cochrane-Review ist nicht vollständig und nicht aktuell, der Bewertungsformalismus ist inadäquat, und die Sachbeurteilungen sind nicht konsistent und oft nicht korrekt. Bei entsprechender Korrektur der Bewertungen kommt man zu einem anderen Gesamtbild der Mistelstudien (zu Details siehe www.misteltherapie.de).

In de Cochrane-review over maretaktherapie (2008) wordt vastgesteld, dat het bestaande wetenschappelijke bewijs ontoereikend zou zijn om de valide toepassing van maretaktherapie te onderbouwen. Deze eindconclusie kan beslist onderschreven worden – de verdere beoordeling, dat het wetenschappelijke bewijs van de onderzoeken over het geheel genomen zwak zou zijn, dient echter kritisch onder de loep genomen te worden.
Over het algemeen kan gesteld worden, dat de Cochrane-review onvolledig en niet actueel is, de beoordelingssystematiek ontoereikend is en de inhoudelijke beoordelingen inconsistent en vaak niet correct zijn.
