Overleving en klachten na percutane aortaklepvervanging
Doel
Het beschrijven van de kenmerken van patiënten met een aortaklepstenose en een hoog operatierisico, die in aanmerking komen voor percutane aortaklepvervanging (PAVR) en het nagaan van het effect van deze behandeling op de overleving en op het optreden van complicaties.
Opzet Prospectief observationeel in één centrum.
Methode
Patiënten met aortaklepstenose en een hoog operatierisico werden gezien tijdens een speciaal spreekuur. Daarna besliste een hartteam of zij in aanmerking kwamen voor PAVR of voor chirurgische aortaklepvervanging (AVR). De onderzoekers registreerden overlijden en complicaties gedurende 30 dagen na PAVR. De studieperiode was september 2005-januari 2009. Met de EuroSCORE werden de kenmerken van de patiënten samengevat, met de kaplan-meiermethode de overleving en met de classificatie van de New York Heart Association (NYHA) de subjectieve toestand.
Resultaten
In de studieperiode werden 166 patiënten verwezen voor PAVR waarvan 91 (55%) daadwerkelijk PAVR ondergingen, 16 (10%) chirurgische klepvervanging (AVR), 39 (23%) werden afgewezen voor AVR/PAVR en 20 patiënten (12%) zagen af van een ingreep. De gemiddelde EuroSCORE (SD) bedroeg bij PAVR 17% (8), bij AVR 12% (7), bij de patiënten die ongeschikt waren voor de ingreep 19% (12) en bij degenen die de ingreep weigerden 25% (14). De mortaliteit en de kans op beroerte 30 dagen na PAVR bedroegen respectievelijk 12 en 13%. De overleving 1 jaar na PAVR was 73%. Dit ging gepaard met een aanzienlijke verbetering van de functionele klasse: 15% van de patiënten had NYHA klasse ≤ 2 vóór PAVR en 72% erna.
Conclusie
PAVR ging gepaard met een aanzienlijk risico op overlijden en beroerte. De subjectieve toestand van de patiënten gaf na PAVR wel een beduidende verbetering te zien. Zolang er geen gerandomiseerd onderzoek beschikbaar is, zijn de auteurs van mening dat PAVR beperkt dient te blijven tot patiënten die niet in aanmerking komen voor chirurgische aortaklepvervanging.

Abstract
Survival and disorders after percutaneous aortic valve replacement
Objective
Description of the characteristics of patients with aortic valve stenosis and high surgical risk considered suitable for percutaneous aortic valve replacement (PAVR) and investigation of the effect of this treatment on survival and the occurrence of complications.
Design Prospective observational single center cohort study.
Method
Patients with aortic valve stenosis and high surgical risk received a special consultation, after which the heart team decided whether they were considered suitable for PAVR or for surgical valve replacement (AVR). Deaths and complications in the 30 day period after PAVR were registered. The study period was September 2005-January 2009. The EuroSCORE system was used to summarise the patient characteristics, survival was determined by using the Kaplan Meier method, and the subjective health status defined following the New York Heart Association (NYHA) classification.
Results
In the study period 166 patients were referred for PAVR of whom 91 (55%) actually underwent PAVR, 16 (10%) underwent surgical valve replacement (AVR), 39 (23%) were rejected for AVR/PAVR and 20 patients (12%) declined surgical treatment. The mean EuroSCORE (SD) for PAVR was 17% (8), for AVR 12% (7) , for patients unsuitable for surgery 19% (12) and for those declining surgery 25% (14). The mortality and risk of stroke 30 days after PAVR were respectively 12 and 13%. Survival at 1 year after PAVR was 73%. This was associated with a significant improvement of the function class: before PAVR 15% of patients had NYHA class ≤ 2, and afterwards 72%.
Conclusion
PAVR was associated with a substantial risk of mortality and stroke. The subjective health status of patients did improve substantially after PAVR. In the absence of randomised studies, the authors are of the opinion that PAVR should be restricted to patients who are not considered suitable for surgical valve replacement.

Een focale leverafwijking na een normaal verlopen zwangerschap
Twee maanden na een normaal verlopen zwangerschap presenteerde een 27-jarige vrouw zich met een focale leverafwijking, die op een CT-scan aanvankelijk geduid werd als 'multipele leverceladenomen'. Twee maanden later bleek het echter om een metastase te gaan van een choriocarcinoom. Dit zeldzame ziektebeeld zal men vaak vergeten bij de differentiaaldiagnostiek van focale leverafwijkingen rond de zwangerschap. Ook omdat het gemetastaseerde choriocarcinoom curatief behandeld kan worden, is het een diagnose die niet gemist mag worden. Daarom is het aan te bevelen om ook de concentratie van humaan choriongonadotrofine (hCG) in het serum te bepalen bij vrouwen in de vruchtbare leeftijd met een focale leverafwijking.

Focal liver lesion after normal pregnancy
Two months after a normal pregnancy, a 27-year old woman presented with a focal liver lesion, which was first interpreted on CT as 'multiple liver cell adenomas'. 2 months later, it turned out to be a metastasis of a choriocarcinoma. This rare neoplasm will often be omitted from the differential diagnosis of liver lesions found during or after pregnancy. However, it is important not to miss this diagnosis since choriocarcinoma metastasis can be successfully treated . Therefore, it is advisable when focal liver lesions are found in women of child -bearing age also to measure human chorionic gonadotropin (hCG) serum concentration.

Fluor vaginalis door groep A-streptokokken bij volwassen vrouwen: een patiënt-controleonderzoek*
Doel
Het verband onderzoeken tussen fluor vaginalis bij volwassen vrouwen en het vόόrkomen in de vaginale flora van bèta-hemolytische streptokokken uit een andere lancefield-groep dan de B-groep.
Opzet
Patiënt-controleonderzoek.
Methoden
De patiënten waren vrouwen met chronische of persisterende fluorklachten waarvan door de huisarts een fluormonster was ingestuurd naar het medisch-microbiologisch laboratorium voor een volledige microbiologische kweek en onderzoek op bacteriële vaginose. De patiënten werden vanuit de reguliere diagnostiek in het onderzoek geïncludeerd. Controlepersonen waren vrouwen zonder klachten die een vaginaaluitstrijk ondergingen, geworven in 4 huisartspraktijken en onder ziekenhuispersoneel. Alle vrouwen waren 30-59 jaar oud en behoorden tot dezelfde huisartspatiëntenpopulatie in de regio Zwolle. De onderzoeksgroepen werden statistisch vergeleken wat betreft de percentages geïsoleerde potentieel pathogene micro-organismen inclusief bèta-hemolytische streptokokken en bacteriële vaginose.
Resultaten
Groep A-streptokokken werden gekweekt uit 49 (4,9%) van de 1010 patiëntmonsters en niet uit de 206 controlemonsters (p < 0,01). De aantallen gekweekte groep C-, -F- en -G-streptokokken waren laag en verschilden niet statistisch tussen de groepen patiënten en controlepersonen.
Conclusie
Bèta-hemolytische streptokokken uit groep A waren geassocieerd met fluor vaginalis bij volwassen vrouwen. Voor een effectieve diagnose van fluor vaginalis is het noodzakelijk een volledige microbiologische kweek uit te voeren als de primaire therapie faalt. De huidige NHG-standaard en overige richtlijnen voor diagnostiek en behandeling van fluor vaginalis zouden hierop moeten worden aangepast.

Vaginal discharge due to group A streptococci in adult women: a case control study
Objective
To investigate the connection between vaginal discharge in adult women and the presence in the vaginal flora of non-group B beta-haemolytic streptococci.
Design
Case control study.
Methods
The patients were women with chronic or persistent discharge from whom a sample for complete microbiological investigation for bacterial vaginosis had been submitted by the general practitioner. The patients were included in the investigation via routine diagnostic procedures. The control group consisted of asymptomatic women who had consented to having a vaginal swab taken, from four medical practices and from hospital personnel. All the women were between 30-59 years old and belonged to the same general practice area in the Zwolle region. The groups were compared statistically in respect of the percentage occurrences of potentially pathogenic micro-organisms including beta-haemolytic streptococci and bacterial vaginosis.
Results
Group A streptococci were isolated in 49 (4.9%) of 1010 cases and not in any of the 206 controls (p < 0.01). Isolation rates of group C, F and G streptococci were low and did not differ statistically between cases and controls.
Conclusion
Group A beta-haemolytic streptococci were associated with vaginal discharge in adult women. For effective diagnosis of vaginal discharge it is necessary to carry out a full microbiological investigation if initial treatment fails. The current NHG-standard and other guidelines for diagnosis and treatment of vaginal discharge should be modified in this respect.
