Cefpodoxim-Proxetil - ein neues Cephalosporin zur oralen Anwendung
Cefpodoxim-Proxetil ist ein Esterderivat. Das Molekül des Antibiotikums weist einige Strukturelemente (z.B. den Aminothiazolrest) auf, die in mehreren parenteral verabreichbaren Cephalosporinen ebenfalls vorhanden sind und diesen Präparaten Stabilität gegen ß-Laktamasen gramnegativer Erreger und hohe Aktivität im gramnegativen Bereich verleihen.
Antibakterielles Spektrum
Cefpodoxim-Proxetil besitzt ein breites antibakterielles Spektrum. Es wirkt gegen grampositive und gramnegative Bakterien und ist stabil gegenüber den häufig vorkommenden ß-Laktamasen. Sowohl die wesentlichen Erreger von Atemwegsinfektionen (Streptokokken, Pneumokokken, Hämophilus influenzae und Moraxella catarrhalis) als auch die häufigsten Verursacher von Infektionen der Harnwege (Escherichia coli, Klebsiella, Proteus mirabilis) werden erfaßt. Die minimalen Hemmkonzentrationen (MHK90-Werte) liegen bei diesen Keimen meist unter 1 mg/l. Staphylokokken sind eher mäßig empfindlich (MHK90-Werte: meist > 2 mg/l). Enterokokken und Pseudomonaden sind resistent (MHK90 > 64 mg/l).
Pharmakokinetische Eigenschaften
Das Prodrug wird nach oraler Gabe im Magen-Darmtrakt resorbiert und durch unspezifische Esterasen zum Wirkstoff Cefpodoxim hydrolysiert. Bei nüchterner Einnahme einer Tablette mit 100 mg Cefpodoxim-Proxetil liegt die Bioverfügbarkeit bei etwa 40 bis 50%. Da die Bioverfügbarkeit durch gleichzeitige Nahrungsaufnahme verbessert wird, sollte das Medikament zu den Mahlzeiten eingenommen werden. Bei gleichzeitiger Einnahme von Präparaten, die den pH-Wert im Magen erhöhen (mineralische Antazida, H2-Antagonisten), wird die Bioverfügbarkeit um ca. 30% reduziert. Die maximalen Plasmakonzentrationen liegen zwei bis drei Stunden nach einer einmaligen Gabe von 200 mg im Mittel bei 2,6 mg/l. Das Verteilungsvolumen beträgt bei jungen Probanden ca. 0,42 l/kg. Das Antibiotikum wird überwiegend unverändert mit einer Eliminationshalbwertzeit von 2,1 bis 2,7 Stunden über die Nieren ausgeschieden. Bei Patienten mit deutlicher Einschränkung der Nierenfunktion ist eine Reduktion der Dosierung notwendig.
Indikationen
Derzeit ist Cefpodoxim nur zur Behandlung von bakteriellen Infektionen der Atemwege zugelassen (Tonsillitis/Pharyngitis, Sinusitis, purulente Bronchitis, Pneumonie). Als Standarddosierung wird die Gabe von zweimal täglich 100 bis 200 mg empfohlen. In verschiedenen Vergleichsstudien war es ebenso wirksam wie herkömmliche Präparate. Klinische Studien zum Einsatz von Cefpodoxim-Proxetil bei Infektionen der Harnwege sowie der Haut und Weichteile stehen kurz vor dem Abschluss. Mit einer Zulassung dieses Präparates auch für diese Indikationen wird dann zu rechnen sein.
Verträglichkeit
Insgesamt war die Verträglichkeit von Cefpodoxim-Proxetil während der klinischen Erprobung gut und war ähnlich, wie die der zum Vergleich herangezogenen Antibiotika. Bei der Auswertung der Daten von etwa 1500 Patienten wurden als unerwünschte Arzneimittelwirkungen hauptsächlich folgende Störungen im Gastrointestinaltrakt beobachtet (Häufigkeit in Klammern): Durchfall (4,6%), weicher Stuhl (0,7%), Übelkeit (1,2%), Bauchschmerzen (1,2%). Juckreiz (0,1%) und Ausschläge (0,48%) traten seltener auf. Schließlich wurden bei einigen Patienten Kopfschmerzen (0,9%) und Schwächegefühl (0,5%) registriert. Es gab keine Hinweise auf eine Beeinflussung der Nieren- oder Leberfunktion oder des Blutbildes durch das neue Medikament. Bei 2,3% der Patienten wurde die Behandlung mit Cefpodoxim-Proxetil wegen unerwünschter Wirkungen abgebrochen (mit Vergleichspräparaten bei 2,2% der Patienten).
ZUSAMMENFASSUNG: Cefpodoxim-Proxetil (ORELOX, PODOMEXEF) ist ß-laktamasefest und besitzt z.B. eine gute Aktivität gegen Streptokokken und H. influenzae. Aus dem Prodrug-Molekül wird während der Resorption aus dem Magen-Darmtrakt die antibakteriell wirksame Substanz Cefpodoxim freigesetzt. Die pharmakokinetischen Eigenschaften erlauben die zweimal tägliche Gabe von 200 mg. Es wird zunächst nur zur Behandlung von Infektionen der oberen und unteren Atemwege angeboten. Hinsichtlich therapeutischer Wirksamkeit und Verträglichkeit bestanden keine wesentlichen Unterschiede gegenüber anderen ß-Laktamantibiotika. Cefpodoxim-Proxetil kann damit als eine neue Alternativ-Substanz zur Behandlung von Infektionen des Respirationstraktes eingeordnet werden.

Cefpodoxime proxetil – a new cephalosporin for oral application
Cefpodoxime proxetil is an ester derivative. The antibiotic molecule has some structural elements (an aminothiazole group, for example), which are also present in several parenterally administerd cephalosporins and give these drugs stability against ß-laktamase from gram-negative pathogens and high activity in the gram-negative spectrum.
Antibacterial Spectrum
Cefpodoxime proxetil has a broad antibacterial spectrum. It is effective against gram-positive and gram-negative bacteria and is stable in the presence of the frequently encountered ß-lactamases. In addition to important pathogens causing respiratory tract infections (streptococci, pneumococci, Haemophilus influenzae and Moraxella catarrhalis) it covers the most common agents that cause urinary tract infections (Escherichia coli, Klebsiella, Proteus mirabilis). The minimal inhibitory concentration (MIC90 values) of these germs was generally below 1 mg/L. Staphylococci are likely to be moderately susceptible (MIC90 values: generally > 2 mg/L). Enterococci and pseudomonads are resistant (MIC90 > 64 mg/L).
Pharmocokinetic Properties
After oral administrations, the prodrug is reabsorbed in the gastronintestinal tract and hydrolyzed by non-specific esterases into the active agent, cefpodoxime. The bioavailability of cefpodoxime proxetil after intake in fasting subjects of one 100 mg tablet ranges from approximately 40 to 50%. As bioavailability is improved by simultaneous intake of food, the medication should be taken with meals. With the simultaneous intake of drugs, which raise the pH-value of the stomach (mineral antacids, H2-antagonists), the bioavailability is reduced by about 30%. Two to three hours after the one time administration of 200 mg, the maximum plasma concentration averages 2.6 mg/L. The volume of distribution in young test persons is approximately .42 L/kg. With an elimation half life of 2.1 to 2.7 hours, the antibiotic is eliminated primarily unchanged via the kidneys. A lower dosage is necessary for patients with significantly reduced kidney function.
Indications
Currently cefpodoxime is onlyapproved for the treatment of bacterial infections of the respiratory tract (tonsillitis/pharyngitis, sinusitis, purulent bronchitis, pneumonia). The standard dosage of 100 to 200 mg – administered twice daily -- is recommended. In various comparative clinical trials it was just as effective as conventional drugs. Clinical studies to determine whether cefpodoxime proxetical can be used in the treatment of urinary tract infections as well as the skin and soft tissue will soon be concluded. Aprroval for these indications is expected upon completion of these studies.
Tolerance
Overall, the tolerance of cefpodoxime proxetil during the clinical trials was good and similar to the antibiotics which were used for comparison. In the analysis of the data of some 1,500 patients the following adverse events of the drug, primarily disorders of the gastrointestinal tract, were observed (frequency in parentheses): diarrhea (4.6%), loose stool (0.7%), nausea (1.2%), stomachache (1.2%). Itching (0.1%) and rashes (0.48%) developed infrequently. Finally, some patients experienced headache (0.9%) and weakness (0.5%). There was no evidence of the new medication influencing kidney or liver function or the blood count. In 2.3% of patients, treatment with cefpodoxime proxetil was discontinued due to adverse effects (in 2.2% of patients taking drugs for purposes of comparison).
SUMMARY: Cefpodoxime proxetil (Orelox, Podomexef) is ß-lactamase resistant and has, for example, good activity against streptococci and H. influenzae. During absorption from the gastrointestinal tract, the antibacterially effective substance, cefpodoxime, is released from the prodrug molecule. The pharmacokinetic properties allow for the twice daily administration of 200 mg. Currently it is only offered for the treatment of upper and lower respiratory tract infections. With regard to therapeutic efficacy and tolerance, there are no significant differences between this antibiotic and other ß-lactamantibiotics. Cefpodoxime proxetil may be classified as a new alternative substance for the treatment of respiratory tract infections.

Swingmusiker im Pop-Paradies
Popularität von Null auf Hundert: Der Sänger Roger Cicero wurde innerhalb kurzer Zeit zum Star. Sehnt er sich manchmal nach dem alten Jazzerleben?
Als Roger Cicero noch Jazzmusiker war, trug er nicht ständig dieses Blues-Brother-Hütchen. Jetzt setzt er die Kopfbedeckung nicht einmal beim Plausch in der Luxus-Lounge seines Plattenlabels ab. Ist das notwendige Corporate Identity eines Stars?
Wer sich mit Cicero unterhält, stellt fest, dass er der sympathische Kumpel geblieben ist, als den ihn seine Jazz-Kollegen kennen: unaufgeblasen-freundlich mit einem Schuss Selbstironie. Dabei genießt er höchste Achtung als Profi, der Stücke arrangiert und ein legendäres Solo des Saxofon-Giganten James Moody als Scat-Sänger virtuos interpretieren kann.
Das war 2005 auf seiner letzten reinen Jazz-CD (mit der Combo After Hours) zu hören. In den vergangenen Jahren gab es Gigs mit der Band Soulounge, ein anspruchsvolles Album mit der Pianistin Julia Hülsmann. "Ich tue, was mir Spaß macht und kann davon leben", so lautete Ciceros Einstellung. Dann erschien 2006 die CD "Männersachen", und der Bigband-Swing mit deutschen Texten machte den inzwischen 37-Jährigen innerhalb von Wochen im ganzen Land bekannt. Obwohl er schon so lange im Geschäft sei, habe er "ein Jahr der ersten Male" hinter sich, sagt der späte Aufsteiger über sein neues Leben und klingt dabei, als sei er selbst noch überrascht.
Das heißt: Statt in kleinen Clubs tritt er nun in großen Sälen auf, bis hin zur Köln Arena mit 6.000 Plätzen; Talk-Shows laden den Musiker ein; Fans schreiben ihm oder bitten auf der Straße um Autogramme - manche sind im Rentenalter, andere gehen noch nicht zur Schule. So imitiert ein Fünfjähriger namens Paul den Sänger im Internet und ist auch schon im Fernsehen aufgetreten: "Der Junge ist fast so berühmt wie ich", sagt Cicero.
Mit dem Titel "Frauen regiern die Welt" gewann Roger Cicero den deutschen Vorentscheid für den Eurovision Song Contest 2007. "Ein Swingsänger mit einer Bigband vertritt sein Land auf Europas größtem Schlager-Festival?", wunderte sich damals ein amerikanischer Jazzmusiker, "da kann man ja wieder an die Menschheit glauben." Dass Cicero 19. unter 24 Teilnehmern wird, tut nichts zur Sache. Swing gilt als popmusikalische Antiquität.
Zu Zeiten Benny Goodmans beherrschte Swing zuletzt die Hitparaden. Aber anders als etwa Bebop hat Swing - nach dessen Rhythmen man tanzen kann - immer wieder Künstler aus dem Showgeschäft fasziniert. Cicero nennt Robbie Williams. Der Ausflug des Popstars in den Bigband-Swing brachte diese Musik einem jungen Publikum nahe und bereitete sicher Ciceros Erfolg in Deutschland mit vor.
Ein Schlüssel zum Erfolg sind aber auch die Texte von Frank Ramond, der auch für Annett Louisan arbeitet. Allerdings ist dem Jazz- und Soul-erfahrenen Cicero das Singen in deutscher Sprache nicht leicht gefallen. "Erst jetzt, nach hundert Auftritten, klinge ich so, wie ich mich kenne", sagt er, "das hört man auf der neuen CD." Das Album mit dem Titel "beziehungsweise" folgt dem Muster der "Männersachen" - pfiffige Texte, swingende Bigband-Arrangements. Es kommt im Oktober auf den Markt; live zu erleben ist Roger Cicero mit den neuen Stücken auf einer Frühjahrstour 2008 mit fast 40 Terminen in den größten Hallen der Republik und Abstechern in die Schweiz und nach Österreich. Er bewältigt das Programm eines Popstars.
Weil er das "aufregend und total ausfüllend" findet, sehnt sich der Cicero bislang nicht nach seinem früheren Leben. Zudem sei er "nicht aus der Jazzwelt weggegangen". So hat er vertraglich sichergestellt, dass er auch mit der Combo After Hours arbeiten darf. Und kürzlich tauchte der neue Star bei einer Jam Session im Hamburger Club "Hörsaal" auf und sang vor 20 Leuten. "Das werde ich öfter machen", verspricht er.
In der Jazz-Szene herrscht denn auch vorwiegend Freude, dass einer von ihnen so grandios aufgestiegen ist. Er könne doch als "Werbeträger für den Jazz" wirken, heißt es über Roger Cicero. Der hat immerhin schon elf Musikanten gut bezahlte Arbeitsplätze geschaffen: Die Mitglieder seiner Bigband kommen alle aus dem Jazz-Milieu.

Popularity ratings zoom from 0 to 100. Faster than a New York minute, singer Roger Cicero, went from relative obscurity to star status. Does he long for the good old days of his former Jazz musician’s life?
While he was still a jazz musician, Roger Cicero, didn’t constantly sport a Blues Brother’s style hat. During our chat in the luxury lounge of his recording label, he never once doffed it. Maybe it’s a necessary fashion accessory for pop stars these days? Anyone engaging Cicero in conversation will soon discover that he’s remained the friendly guy that his jazz colleagues know him to be. Along with a winning personality -- a combo of down-to-earthiness with a dash of self-deprecating humor -- he enjoys the highest professional regard: known for his ability to arrange and lay down a virtuoso scat rendition of the legendary sax solo of jazz giant James Moody.
That rendition appeared on his last pure jazz CD (with the jazz combo After Hours). Through the years there have been gigs with the group Soulounge and a critically acclaimed CD with jazz pianist , Julia Hülsmann. You might say Cicero’s M.O. was: “I do whatever’s fun and pays the rent.” Then came the 2006 release of “Männersachen”; and within a matter of weeks, the disc with the Big Band Swing and German lyrics rocketed the now 37 year old to stardom. Even the late-blooming Cicero himself seems incredulous: after being in the business for most of his life, he now has a year of “firsts” behind him.
This means instead of performing in small clubs he’s now headlining at big venues, like the 6,000 seat Cologne Arena; talk shows are lining him up; he’s getting fan mail and being stopped on the street for autographs. His fan base runs the gamut from retired folks to preschoolers. There’s a five year old Cicero wannabe, by the name of Paul, doing impersonations of him on the Internet -- he’s even performed on TV. “The kid is almost as famous as me,” quipped Cicero.
With the song “Frauen regieren die Welt”, Cicero represented Germany in the 2007 Eurovision Song Contest -- a European institution since the mid-50s. Participating countries (this year there were 42) each enter a song which is then judged. “A swing singer, with a big band, representing his country in Europe’s biggest Popfest,” mused an American jazz musician who attended the event “that could restore your faith in humanity.” That Cicero placed 19th out of 24 performers was beside the point. Swing belongs in the realm of pop music antiquity.
The last time Swing dominated the Top 40 charts was during the reign of Benny Goodman. But unlike Bebop, Swing -- which has a danceable beat -- has always held a certain fascination for showbiz performers. Cicero acknowledges British pop star, Robbie Williams, whose venture into the world of Big Band brought the music to the attention of “Generation Next” and certainly paved the way for Cicero’s success in Germany.
Certainly one key to Cicero’s success are the lyrics of Frank Raymond who also writes for German singer, Annett Louisan. Even with the aid of the first rate lyrics, singing in German hasn’t come easy for this experienced jazz singer. “It’s only now after 100 performances that I’m beginning to sound like me; and you can hear that on the CD,” said Cicero. The CD, “beziehungsweise”, continues on in the same vein as “Männersachen”: catchy lyrics, Big Band arrangements that groove. It’s due for release in October. He’s now ramping up for his 2008 spring tour featuring the new material. Nearly 40 concerts are slated for Germany’s biggest venues with side trips to Switzerland and Austria. Cicero owns the pop circuit.
Because he’s finding all of this “exciting and totally fulfilling”, Cicero’s not looking back wistfully on his former life. In any case, he hasn’t “dropped out of the Jazz scene”. To that end, he has contractually safeguarded his right to continued collaboration with After Hours. And a short while ago he popped up at a jam session in the Hamburg club “Hörsaal” where he sang for an audience of 20. “I’ll be doing a lot more of that,” he promised. From his peers in the jazz world, a general feeling of good will prevails towards the cat that that hit the big time. Word on the street has it that Cicero could be the next poster child for Jazz. At the end of the day, his good fortune has extended to 11 of his fellow jazz musicians who are now well paid members of his Big Band.
