L’anémie est définie par une diminution de la masse sanguine entraînant une diminution de I'hématocrite et du taux d'hémoglobine. Classiquement, son diagnostique est posé devant un taux d’hémoglobine inférieur à 12 g/dL chez la femme et inférieur à 13 g/dL chez l’homme. L'anémie et la fatigue qui en découle sont des complications classiques de la maladie cancéreuse. L’anémie est fréquente dès le diagnostic, elle est retrouvée chez environ 30 % des patients cancéreux avant tout traitement, La fatigue constitue une plainte majeure des patients, mais souvent négligée par les médecins qui les prennent en charge.
Incidence chez les patients atteints de cancers pulmonaires
La fréquence de I'anémie dépend du stade, de l’évolution de la maladie cancéreuse, des traitements reçus et de la localisation initiale de la tumeur [6]. Ainsi, les patients atteints de cancer pulmonaire sont plus affectés que les autres par I'anémie. On estime que 50 a 70 % d'entre eux en souffriront durant leur maladie. Ces chiffres sont à comparer aux 15-20 % de patients anémiques atteints d'un cancer colorectal. L'anémie au cours du cancer pulmonaire est non seulement fréquente mais aussi sévère avec jusqu'à 55 % de patients présentant une anémie menaçante (Hb < 6.5 g/dL) dans certaines séries.
Corollaire de cette forte incidence d'anémie, le taux de transfusion est particulièrement élevé dans cette population. Ainsi, dans le cancer du poumon, 34 a 43 % des patients sont transfusés au cours de leur maladie contre 13 à 19 % des patients traités pour cancer du sein. De plus, 22 % des patients seront polytransfusés en cas de cancer du poumon, 7 % seulement en cas de cancer du sein.
Traitement
Le retentissement de I'anémie, son incidence sur le choix des traitements, son impact pronostique sont autant d'arguments pour un traitement efficace, avec pour but I'amélioration de la qualité de vie au moins, une meilleure efficacité des thérapeutiques spécifiques, voire un gain en survie au mieux, si tant est que la correction de I'anémie puisse avoir un tel impact. Les options thérapeutiques à notre disposition aujourd'hui sont la transfusion de culots globulaires et (l’administration d'érythropoïétine recombinante. Les traitements à visée symptomatique sont aussi importants : oxygénothérapie, adaptation du traitement d'une bronchopathie chronique, d'une cardiopathie ischémique, soutien nutritionnel et psychologique, traitement antidépresseur. lls doivent accompagner le traitement de I'anémie.
La transfusion est indiquée de façon consensuelle et évidente dans les situations d'anémie sévère exigeant un retour rapide à une hémoglobinémie proche de la normale. Toutefois, elle n'est pas réservée aux anémies sévères. Pourtant, la majorité d'entre elles sont encore réalisées lorsque I'hémoglobinémie est inférieure à 8 g/dL ou que les patients se plaignent d'une symptomatologie sévère ou présentent une complication. Les risques transfusionnels infectieux, I'immunodépression post-transfusionnelle, les risques allergiques et la pénurie de produits sanguins sont des critères pour limiter les transfusions. Si ces critères sont justifiés concernant la limitation du nombre de transfusions, ils ne s'appliquent pas aux autres traitements de I'anémie et ne peuvent donc expliquer son traitement inadéquat.
De plus, il est maintenant bien établi que le traitement de I'anémie légère, en restaurant une hémoglobinémie normale ou quasi-normale, permet la disparition de tous les symptômes de I'anémie et amène un gain en qualité de vie supérieur à celui obtenu par la correction incomplète d'une anémie sévère. L'amélioration la plus importante survient lors du passage de 11 à 12 g/dL d'hémoglobine. Un traitement précoce de I'anémie est donc justifie, avant le passage du seuil de 10 g/dL et le recours aux transfusions. L'état général médiocre et la mauvaise tolérance de I'anémie des patients atteints d'un cancer du poumon sont des arguments supplémentaires pour un traitement actif et précoce.
Le traitement de I'anémie par EPO recombinante est efficace pour une majorité de patients cancéreux, et ce quelle que soit la molécule utilisée, la localisation initiale du cancer ou la chimiothérapie administrée.
Conclusion
L'anémie est fréquente chez les patients atteints de cancer pulmonaire. Elle est souvent négligée et non traitée. Pourtant, elle est delétère pour la qualité de vie des patients et peut limiter les résultats positifs des traitements anticancéreux. II existe différentes options thérapeutiques validées permettant un gain évident en qualité de vie. Nous avons montré ici (l’importance particulière du traitement de I'anémie pour la prise en charge optimale des patients atteints de cancer du poumon. La recherche clinique en soins palliatifs doit se développer pour répondre au plus vite aux questions encore posées concernant cette problématique.

Anaemia is defined as a decrease in blood volume leading to a decrease in haematocrit and haemoglobin concentration. The classical criteria for diagnosis of anaemia are when the haemoglobin level is less than 12 g/dl in women and less than 13 g/dl in men. The anaemia and the fatigue which results from it are standard complications in cancer. Anaemia occurs frequently from the time of diagnosis and it is found in about 30% of cancer patients before any treatment. Fatigue is a major complaint of patients, but is often neglected by treating physicians.
Incidence in patients with lung cancers
The frequency of anaemia depends on the stage, the development of the cancer, treatment received and the original site of the tumour. Thus, patients with lung cancer are more affected than others by anaemia. It is thought that 50 to 70% of them will suffer from it during their illness. These figures should be compared with 15-20% of colorectal cancer patients with anaemia. Anaemia during lung cancer is not only frequent but is also severe with up to 55% of patients presenting with severe anaemia (Hb < 6.5g/dL) according to some data.
As a result of this high incidence of anaemia, the transfusion rate is particularly elevated in this patient population. Thus, in lung cancer, 34 to 43% of patients are transfused during their illness compared to 13 to 19% of patients treated for breast cancer. Furthermore, 22% of patients with lung cancer will undergo repeated transfusions compared to only 7% with breast cancer.
Treatment
The effects of anaemia, its impact on the choice of treatments and its prognostic influence serve as arguments for an effective treatment. The aim of this would be to improve the quality of life with better effective specific therapies and at best achieve an increase in survival rate, that is if the correction of anaemia can have such an impact. The therapeutic options available today are transfusion of packed red blood cells and the administration of recombinant erythropoietin. Treatments designed for symptomatic control are also significant and include: oxygen therapy, adapting the treatment for chronic lung disease and for ischaemic cardiac disease, nutritional and psychological support and antidepressant therapy. These should be given in conjunction with the treatment for anaemia.
Transfusion is indicated in cases of severe anaemia by clear common consensus and this requires a rapid return to a near-normal haemoglobin level. However, it is not reserved for severe anaemia. The majority are however still carried out when haemoglobin is lower than 8 g/dL or when patients complain of severe symptoms or have a complication. The risks of infections associated with the transfusions, post-transfusion immunosuppression, risks of allergy and the shortage of blood products are factors for limiting transfusions. Justified as these factors may be as regards limiting the number of transfusions, they do not apply to other treatments for anaemia and cannot, therefore, explain its inadequate treatment.
Furthermore, it is now well-established that the treatment for low-grade anaemia allows all the symptoms of anaemia to disappear by restoring a normal or near-normal haemoglobin level and brings an increase in quality of life superior to that obtained by the inadequate correction of severe anaemia. The greatest improvement occurs when the haemoglobin level increases from 11 to 12 g/dl. Early treatment of anaemia is therefore justified before reaching the 10 g/dL threshold and resorting to transfusions. Poor general health and the poor tolerance of anaemia in patients with lung cancer are additional reasons for active and early treatment.
Treatment for anaemia by recombinant Erythropoietin (EPO) is effective for the majority of cancer patients, regardless of which molecule is used, the primary site of the cancer or the chemotherapy administered.
Conclusion
Anaemia is frequent in patients with lung cancer. It is often ignored and untreated. However, it is detrimental to patients’ quality of life and can restrict the positive results of anticancer treatments. There are various proven therapeutic options which enable a marked improvement in quality of life. We have demonstrated here the particular importance of treating anaemia for the optimal care of lung cancer patients. Clinical research in palliative care should be developed so as to deal as quickly as possible with unsolved questions regarding this issue.
