2 OBLIGATIONS OF THE OPERATION & MAINTENANCE (O&M) CONTRACTOR
The O&M Contractor acknowledges that it shall provide operating and maintenance personnel and other support services necessary to perform its obligations set forth in the Scope of Supply for the management, operation and maintenance of the facility, including without limitation the following obligations:
2.1 Operating and maintaining the Facility in accordance with the Power Purchase Agreement, and the practices, standards and procedures customary in the nuclear power industry
2.2 Obtaining and maintaining insurance as required to be obtained and maintained by the under the Agreement without limiting the rights of the Owner or EPC Contractor to provide independently for its own additional insurance coverage (if any)
2.3 Maintaining complete records of accounts and of technical operations of the facility and preparing all reports, statements, data and information that may be required from time to time under the Agreement or by any governmental authorities, and retaining records that are not required as permanent records, for a period of at least three years after the end of such year
2.4 Opening and maintaining bank accounts and performing cash management functions in connection with the operation of the Facility, including, and the payment of all costs, expenses, rentals and taxes incurred

2 ОБЯЗАННОСТИ ЭКСПЛУАТИРУЮЩЕЙ И ОБСЛУЖИВАЮЩЕЙ ПОДРЯДНОЙ ОРГАНИЗАЦИИ
Эксплуатирующая и обслуживающая подрядная организация подтверждает, что она предоставит эксплуатационно-обслуживающий персонал и другие вспомогательные службы, необходимые для выполнения своих обязательств в соответствии с оговоренным Объемом поставки Услуг по управлению, эксплуатации и обслуживанию установки, включая, но не ограничиваясь следующими обязательствами:

ALGERNON REGINALD ST. CLAIR DE BONKERS fell to the floor of the bathroom with a soggy scrunch. Algernon lay upon the floor and from him there came bubbling, mewling sounds. Out in the corridor a chambermaid who was passing stopped in her tracks and felt the icy fingers of fear crawl up and down her spine. Tremulously she called through the door, ‘Are you all right, Sir Algernon? Sir Algernon, are you all right?’ Receiving no reply she turned the door handle and entered the bathroom.
Immediately her hair stood up on her neck, and drawing a tremendous breath she let go with the most marvelous scream of her career, and continued to scream, getting higher and higher up the scale as she did so. Thoroughly out of breath, she collapsed in a dead faint by the side of Algernon on the floor.
There came the sound of excited voices. There came the sound of pounding feet up the stairs and along the corridor.
The first-comers stopped with such abruptness that they tore the carpet from its fastening, then clustered together as if to give each other confidence they peered in the open door-way.
Algernon Reginald St. Clair de Bonkers lay upon his face on the bathroom floor, blood pouring from a gash across his throat and soaking the unconscious body of the chamber-maid lying beside him. Suddenly she took a quick gasp,
twitched, and opened her eyes. For seconds she looked at the pool of blood beneath her, shuddered, and then with an eldritch scream which jarred the nerves of those around she

Type 2 Diabetes
Type 2 diabetes (T2D) is one of the fastest growing health concerns worldwide. The number of
people with all types of diabetes is expected to rise from approximately 171 million in the year
2000 to 366 million in 2030, primarily due to population growth, aging and increasing prevalence
of obesity and physical inactivity. T2D accounts for 90%-95% of all cases of diabetes in patients
over 20 years of age and occurs more frequently in adults. It is also being found increasingly in
adolescents.
T2D is a chronic metabolic disease characterized by hyperglycemia resulting from disorders of
insulin action and/or insulin secretion. As the disease progresses, most patients develop additional
comorbidities, including macrovascular disease, nephropathy, retinopathy, and neuropathy. The
risk of developing cardiovascular disease is 2- to 4-fold higher in T2D patients than in the non-
diabetic population.
Diabetes-related complications can be prevented or delayed by achieving tight glycemic control.
However, it is increasingly being recognized that the management of associated cardiovascular
risk factors, particularly dyslipidemia, is equally important for a substantial reduction in T2D-
related morbidity and mortality.
Non-Clinical Toxicology
Single and multiple oral and intravenous dose toxicity studies were performed in male and female
mice, rats and monkeys. Embryo-fetal development studies in rats and rabbits and a reproductive
toxicity study in rats have been completed.
At high doses (≥ 300 mg) compared to the expected therapeutic dose range in humans
(~ 5-20 mg) effects considered to be of most importance were signs of fluid accumulation, such as
marked subcutaneous edema or ascites, and histopathological findings in the heart. The heart
findings consisted of myocardial hypertrophy in rats and mice and interstitial expansion between
cardiomyocytes and myodegeneration in monkeys. In addition, two mild and reversible dose-
dependent effects were observed only in monkeys in the 6-month multiple dose study: renal single
cell necrosis in the proximal tubules and subendocardial cardiac microhemorrhage evidenced by
presence of erythrocytes.
Effects observed up to plasma exposures of 500 ng·h/mL were minimal and reversible. They were
not considered to be dose-limiting for human studies because they have previously been reported
for other drugs of this class without any major adverse consequences of clinical relevance in
humans.
Some inconsistent effects were observed on blood cells at high dose levels in rats and monkeys,
but as these were generally minor and values were well within the normal range, these effects
were thus not considered to be of toxicological relevance.
Concomitant Medication (Drugs and Therapies)
Concomitant medication refers to drugs used during participation in the study, e.g. during or after
study medication intake. The Investigator should record the use of all concomitant medications
during the last 3 months prior to visit 1 in the CRF. Subjects should be discouraged from starting
any new medication, both prescribed and over-the-counter, without consulting the Investigator,
unless the new medication is required for emergency use, such as in the management of adverse
events.
Subjects are allowed to continue taking their medication for concomitant diseases, when the
medication is on a stable level during the study, with the exception of insulin, oral blood glucose
lowering drug and loop-diuretics.
If the subject’s health condition requires the use of other medication during the study, then he/she
will be withdrawn from the study, except where it is concluded that the required medication is not
expected to influence the outcome of the study in any way. This will be discussed between the
Investigator and the Study Manager or designees. | Translation - Russian
Диабет 2 типа
Диабет второго типа (Д2Т) одно из наиболее быстро распространяющихся заболеваний в мире. Ожидается, что число людей, больных диабетом всех типов, возрастет с почти 171 млн. в 2000 г. до 366 млн. в 2030 г., в основном по причине роста и старения населения, а также увеличения числа людей, страдающих ожирением и недостатком физической активности. В 90-95% случаев Д2Т встречается у пациентов старше 20 лет, и чаще всего он возникает у взрослых. А также его все чаще находят у подростков.
Д2Т — это хроническое нарушение обмена веществ, которое характеризуется гипергликемией в результате действия и /или секреции гормона инсулина. По мере прогрессирования заболевания у большинства больных развиваются дополнительные сопутствующие недуги, включая макрососудистые заболевания, нефропатию, ретинопатию и невропатию. Риск развития сердечнососудистых заболеваний у пациентов, страдающих Д2Т в 2 - 4 раза выше, чем у людей, не страдающих диабетом.
Возникновение связанных с диабетом осложнений можно предотвратить или задержать посредством строгого контроля над уровнем глюкозы в крови. Однако врачи все чаще признают, что для существенного сокращения заболеваемости Д2Т и смертности не менее важна профилактика сердечнососудистых заболеваний , особенно дислипидемии .
Неклиническая токсикология
Исследования токсичности одиночных и многократных доз для перорального и внутривенного применения были проведены на мышах, крысах и обезьянах обоих полов. Исследования эмбрио- и фетотоксичности проводились на крысах и кроликах, а исследования репродуктивной токсичности осуществлялись на крысах.
При применении высоких доз (> 300 мг), в сравнении с предполагаемым диапазоном лечебных доз для человека (~ 5-20 мг), наиболее существенными были признаки скопления жидкости, так например, отмечался подкожный отек или асцит, и гистопатологические изменения в сердце. У крыс и мышей изменения заключались в гипертрофии миокарда, а у обезьян — в увеличении интерстициального пространства между кардиомиоцитами и дегенерации мышц. Кроме того, только у обезьян, принимавших многократные дозы на протяжении 6 месяцев, наблюдались два легких и обратимых зависимых от дозы эффекта: отмирание отдельных клеток почек и субэндокардиальное микрокровотечение, на которое указывало наличие эритроцитов.
Эффекты, наблюдавшиеся при уровне содержания в плазме до 500 нг-х/мл, были минимальными и обратимыми. Они не могут служить причиной ограничения дозы для человека, поскольку ранее уже отмечались у других препаратов этого класса без каких-либо серьезных негативных клинических последствий.
При применении высоких доз у крыс и обезьян наблюдались отдельные непостоянные эффекты в клетках крови, но так как они были незначительны и их показатели находились в пределах допустимой нормы, то с точки зрения токсикологии их нельзя считать актуальными.
Сопутствующая терапия (лечение и препараты)
К сопутствующим терапевтическим средствам относятся препараты, применяемые в ходе исследования, например, во время или после исследования. В процессе исследования нужно вести учет применения всех сопутствующих препаратов за последних три месяца до даты 1-го визита, указанной в индивидуальной регистрационной карте (CRF). Испытуемые должны воздерживаться от приема любых новых препаратов, приобретенных по рецепту или в свободной продаже, без предварительной консультации с врачом-исследователем, если только прием такого препарата не вызван острой необходимостью, например, для устранения побочных эффектов.
Испытуемым разрешается продолжать прием препаратов для лечения сопутствующих болезней, если дозировка таких препаратов является постоянной весь период исследования, за исключением инсулина, перорального приема сахароснижающих препаратов и петлевых диуретиков.
Если в процессе исследования испытуемый, по состоянию здравья, нуждается в приеме других препаратов, его нужно отстранить от обследования, кроме случаев, когда прием таких препаратов не повлияет на результаты исследования. В таких случаях врач-исследователь должен обратиться за консультацией к руководителю исследования или назначенному должностному лицу.
