Per ottenere il buon funzionamento e una lunga sopravvivenza di un impianto protesico, è necessario che si verifichi la fissazione completa dell’impianto stesso all’osso del paziente. L’assenza di micromovimenti delle componenti protesiche, o quantomeno micromovimenti che siano inferiori ai 20 micron nei primi due mesi dall’intervento, insieme al contatto fra l’impianto e l’osso ospite per una superficie quanto più ampia possibile, sono fattori indispensabili per ottenere una osteointegrazione ottimale della protesi.
L’utilizzo della componente femorale protesica tradizionale prevede il sacrificio della testa e del collo femorale del paziente. In seguito allo sviluppo della chirurgia protesica e con l’estensione delle indicazioni all’applicazione delle protesi in pazienti sempre più giovani, è nata l’esigenza di risparmiare quanto più osso possibile, soprattutto in previsione di un futuro intervento di revisione dell’impianto. A tal proposito sono state sviluppate protesi che prevedono la conservazione del collo femorale e protesi che consentono la conservazione sia del collo che della testa del femore.
Le problematiche maggiori legate all’utilizzo di steli femorali a conservazione del collo sono la mobilizzazione e l’insorgenza del fenomeno di stress-shielding, ovvero il riassorbimento osseo periprotesico a livello del femore prossimale, fenomeno capace di compromettere la stabilità e la durata nel tempo dell’impianto protesico.
| Translation - English THE PROSTHETIC IMPLANT
Concept, experimental results, indications
The longevity and good function of a prosthetic implant depends on the complete fixation of the implant itself to the patient’s bone. The absence of micro movements on the prosthetic components, or micromovements which are at least less than 20 micron during the first two months after the operation, together with the bone-to-implant contact on the largest possible area, are essential elements in order to obtain an optimal osseointegration of the prosthesis.
The use of the traditional femoral prosthesis implies the loss of the patient’s femoral head and neck. As consequence of the development of prosthetic surgery and of the growing demand of prosthesis application to younger and younger patients, it is necessary to save as much bone as possible, especially in expectation of a future operation for the implant checkup. For this reason, a prosthesis has been developed to preserve the femoral head and another prosthesis has been developed to preserve both the femoral head and neck.
The major problems linked to the use of femoral stems, used to preserve the neck, are mobilization and the manifestation of the stress-shielding phenomenon, which is the osseo periprosthetic resorption of the femoral bone, and which is a phenomenon that can compromise the stability and longevity of the prosthetic implant.
