Introduction
Patients suffering from chronic ischemic wounds often have multiple chronic conditions that impair wound healing. We present two cases we treated with a new therapy working with oxygen and cyclical pressure.
Methods and Results
Topical Wound Oxygen (TWO2) from the manufacturer AOTI Ltd, Ireland works with purified oxygen and pressure cycles between 5 and 50mbar to enhance the partial oxygen pressure in the wound tissue.
Case 1:
A 64 year old male patient had an autologous femoro-popliteal bypass surgery done 4 weeks prior to admission. We saw the patient with a complete necrosis of the skin on dorsal site of the foot. Surgical removal of necrosis and resection of compartment on back of foot as well as amputation of toes were performed. We continued therapy with negative pressure therapy (NPT) and intermittent TWO2 therapy. After skin grafting NPT and intermittent TWO2 therapy for 7 days was done. After stopping NPT, TWO2 treatment alone was for 10 days before dismissal home.
Case 2:
72 year old male diabetic patient with AVK developed a gangrenous forefoot. Prior to admission to surgical ward therapy with prostavasin was done. There were no possibilities to improve arterial vascular status via surgical procedures. A transmetatarsale amputation with repeated debridements was performed followed by negative pressure therapy and resesection of necrotic tissue. After 6 weeks of no further improvements we started with TWO2 for 6 days. The wound granulated well and we decided to skin graft followed by negative pressure and TWO2 therapy on days of dressing changes for 6 days. After 13 days of solely TWO2 the wound granulated well and the patient was dismissed to rehabilitation.
Conclusion
In these two complicated cases both associated with severe co morbidities TWO2 proved to be an valuable adjunctive therapy with good results in healing and more important to maintain the functional status by avoiding major amputation.

Introduction
Les patients souffrant de plaies chroniques ischémiques présentent souvent des conditions chroniques multiples qui freinent la guérison des plaies. Nous rapportons deux cas que nous avons traités en utilisant une thérapie par oxygénation et pression cyclique.
Méthodes et Résultats
La thérapie par oxygénation locale des plaies (Topical Wound Oxygen- TWO2), proposée par le fabricant irlandais AOTI Ltd, combine l’utilisation d’oxygène purifié et de cycles de pression d’entre 5 et 50mbar afin d’amplifier la pression partielle de l’oxygène dans les tissus de la plaie.
Cas 1:
Un patient âgé de 64 ans et de sexe masculin subit une opération chirurgicale de pontage autologue fémoro-poplité 4 semaines avant admission. On observa une nécrose complète de la peau sur la face dorsale du pied. On procéda à l’ablation chirurgicale des tissus nécrosés, à la résection du compartiment sur la face dorsale du pied, ainsi qu’à l’amputation des orteils. On prolongea le traitement par une thérapie par pression négative (TPN) et par une thérapie intermittente de TWO2. Après la greffe de peau, on continua d’appliquer la TPN et la TWO2 en intermittence pendant 7 jours. La TPN fut alors interrompue et l’on prolongea seulement le traitement TWO2 jusqu'à la fin de l’hospitalisation.
Cas 2:
Un patient diabétique, âgé de 72 ans et de sexe masculin, subit une amputation mi-cuisse et développa une gangrène de l’avant-pied. Avant son admission au service chirurgical, un traitement à base de prostavasine fut prescrit. On nota l’impossibilité d’améliorer le statut artério-vasculaire chirurgicalement. On procéda à une amputation transmétatarsienne avec débridements répétés, suivie d’une thérapie par pression négative et de la résection des tissus nécrosés. Ne notant aucune amélioration après 6 semaines, la TWO2 fut administrée pendant 6 jours. On observa une granulation effective de la plaie et procéda alors à la greffe de peau, puis à la TPN et à la thérapie TWO2 les jours de changements de pansements, durant 6 jours. Après 13 jours de thérapie TWO2 seule, on observa une continuation de la granulation de la plaie et le patient put être déshospitalisé.
Conclusion
Dans ces deux cas complexes impliquant des co-morbidités graves, la TWO2 s’est révélée être une thérapie adjonctive efficace, permettant d’obtenir des résultas positifs de guérison et a notamment permis le maintien du statut fonctionnel, empêchant ainsi une amputation majeure.
