Source text - Italian Case Report A 54-year-old woman with longstanding history of systemic arterial hypertension was admitted to our emergency department with a clinical picture of Acute Pulmonary Oedema (APE). Arterial systemic blood pressure and heart rate were respectively 150/80 mmHg and 120 bpm. On auscultation an accen-tuated first heart sound with a diastolic apical rumble were heard and rales?? were present on the mid-basal pulmonary fields bilaterally. Trans-cutaneous (hypodermic??) oxygen saturation was 84%. ECG demonstrated sinus tachycardia with signs of left ventricular hypertrophy (LVH). After an initial treat-ment with digitalis, diuretics, morphine and oxygen, the patient was transferred to Coronary Care Unit, where a trans-thoracic bi-dimensional echo-cardio-graphic colour Doppler exam (TTE) revealed a huge, ovoid, homogenously hyper-echoic, peduncolated mass adherent to the caudal portion of the left side of inter-atrium septum, compatible with the diagnosis of myxoma (Fig 1 and 2). The mass was found prolapsing into the left ventricle (LV). During diastole it occluded the mitral valve orifice almost com-pletely. Mild LVH with normal global and regional systolic function were documented. LV filling flow was characterized by only a small diastolic turbulent jet. Pulmonary veins were not obstructed. Pulmo-nary artery systolic pressure (PASP) was elevated. Mild posterior pericardial effusion (PE) was de-tected. It is worthy of mention that a TTE performed 20 months before showed no remarkable findings except for mild LVH and mild dilatation of the left atrium (LA). The patient underwent preoperative coronary angiography which revealed normal coronary artery and was immediately transferred to the surgical department for excision of the left atrial mass. The operation was performed median opening of the sternum under cardiopulmonary bypass, using mild hypothermia (33°C) and hyperkaliemic warm blood cardioplegia. Transept approach to the LA allowed resection of a 7x3 cm mass. Histologi-cal analysis of the lesion demonstrated typical microscopic features of an atrial myxoma (Fig 3). The postoperative recovery was uncomplicated and the patient was discharged home on the seventh postop-erative day. Pre-discharge TTE showed no residual mass in the LA, complete resolution of the pulmo-nary hypertension (PASP=25 mmHg) and only a mild PE. At six months follow-up the patient re-mained asymptomatic and TTE was unchanged. Asthma course worsens in one third, improves in one third, or remains unchanged in one third of women during pregnancy. Pregnant asthmatic women have an increased risk of perinatal mortality, preeclampsia, low birth weight infants, and preterm births compared to non-asthmatic women. Uncontrolled asthma increases perinatal risks while controlled asthma reduces these risks. Objective assessments and monitoring, including pulmonary function testing (ideally spirometry), detailed symptom history, and physical examination should be performed on a monthly basis in pregnant asthmatic women with persistent asthma. In patients starting inhaled corticosteroids during pregnancy, budesonide has been recommended as the inhaled corticosteroid of choice. Current guidelines recommend a stepwise approach in achieving and maintaining asthma control with medications during pregnancy. Source text - English In the management of patients with difficult to control asthma, one special situation that one must consider is the case of those individuals with asthma and eosinophilia who fulfill the diagnostic criteria of Churg-Strauss syndrome (CSS). CSS is a relatively rare multisystem disorder that is manifested by airway obstruction, pulmonary infiltrates, systemic eosinophilia, sinusitis, neuropathy, constitutional symptoms and eosinophilic vasculitis of several organs including the lungs, heart, gastrointestinal (GI) tract, and kidneys. Untreated, CSS may have a dire prognosis, but treatment with corticosteroids and/or other cytotoxic agents usually results in clinical remission of the disease. Thus, it is important to establish a tissue diagnosis and exclude other diseases in the differential diagnosis as treatment options differ significantly from other eosinophilic lung diseases. While the eosinophil is felt to play a central role in CSS pathogenesis, the etiology of this syndrome remains unknown. Come è noto, nell’insieme di rappresentazioni dello spazio geografico praticate nell’antichità classica possono essere individuati due distinti modelli. Il primo, altamente elaborato, è quello cartografico-geografico “tolemaico”: fondato su di una precisa concezione del cosmo, della quale è parte integrante il sapere astronomico ed astrologico; accompagnato da un rigoroso sapere geometrico-matematico, espresso anche nelle tre proiezioni tolemaiche.	Translation - English This contribution is inscribed within an unpublished research project around what might be termed ‘odological design’. This is a project which encompasses elaborating an IT repertoire apt to include, catalogue, allow comparison of very different cartographic experiences and which may be ascribed to a model whereby the traveller traverses spaces illustrated through drawing. As is known, within the representations of geographical spaces taken together in ancient classic times two distinct models can be apprehended. The first, by far the most studied, is the cartographic-geographic Ptolemaic one: this is founded on a precise cosmogony, which the astronomical and astrological is part and parcel to. This is also accompanied by a rigorous geometrical-mathematical awareness, which is also expressed in the three Ptolemaic projections.

In Galilei’s times, this was a novelty. In such an uncomfortable position, the only way towards self-identification he could find was that of a Lincei Academician (from the lynch, an animal celebrated for its acute sight). Truthfully, these youths too were near the border with ‘natural magic’ – the last of the great Reinassance magicians, Giovan Battista della Porta, had been among the very first members of Academy, but Galilei’s instruments (telescope and microscope) to lean towards interests for a naturalistic observation of the marvels of natural world.
