Hello and welcome. My name is Julie DiCarlo. I'm the Scientific Director of Arthritis
Imaging at Synarc, and today we're going to be talking about some guidelines for
radiographic examinations of the hands, wrists, and feet, for this Pfizer protocol.
We're really excited, at Synarc, to be partnering on the program evaluating the
therapy; and those of you who see patients with RA in your clinics on a daily basis
know how debilitating this disease can be. The fact that this therapy has the
potential to really bring relief to patients, and as an oral therapy is truly exciting for
us, and we are really excited about participating in this program.
Today we're going to go over some techniques we can use together to ensure we have
the best quality images possible

So first, an overview.
We're going to talk about the responsibilities, and how the image analysis works; we'll
go over some specific instructions on how to perform the X-rays to ensure we have the
best images possible for the assessments; and there are a few things that we do
differently in clinical trials, where changes between subsequent visits have to be
scrutinized in ways that are different from typical clinical imaging.
Finally, we're going to go over some technical equipment requirements, and some
procedures for sending us the data

So the role of radiographs in this study is going to be to help determine the effect of
the treatment on structural joint damage, in subjects with RA.
There are two things that are going to be scored on the radiographs, and those are
erosion and joint-space narrowing, and I've got a couple of images in a few slides that
will show you exactly how these images are going to be scored.

It's really important to remember that high image quality is necessary for accurate
staging, and evaluating the progression of the disease. Changes in structural joint
damage in subjects with RA are going to be used as primary and secondary endpoints
for this study.
And throughout this presentation we're going to focus on the need for consistency,
because any variabilities in how the X-rays are acquired could actually mask as
structural change and give an unfair assessment of the treatment.

So let's talk about how we standardize the radiographic technique for a clinical trial.
First, we do a centralized training like this one.
You should have—or should receive soon—an X-ray procedure manual that gives a
detailed description of the procedure, and will be a full reference for you to use once
this talk is over, and throughout the duration of the study.
Finally, we will provide telephone support, so that at any time, if you have questions,
you can contact one of our in-house radiographic technologists.

Once you've read through the manual, you'll see there's a sign-off form. We would
like you to fill this out and send it back to us. This will acknowledge that you have
read, and understand, all of the procedures and responsibilities that are detailed.
We need sign-offs for the study coordinator, for the lead technologists, and any other
technologists who are going to be acquiring images.

So what are Synarc's responsibilities in this study?
We're going to be collecting the film; we're going to be performing quality control of
the images; and finally, our radiologists will be performing central reading of the
hand and foot radiographs.

The quality control of the images we get are going to be [sic] checking various
parameters, like collimation, beam centring, making sure the Syn-X-RA, our standard
positioning device, is used—we'll talk about that a little bit later, making sure all the
anatomy is properly depicted, and checking that the scale is present on digital films,
and that for digital images the pixel size is accurate.

What are your responsibilities as the X-ray technologist?
You're going to be performing the exams of the hands, wrists, and feet, and we really
depend on you to review the quality of the images, and obtain a repeat if you think
it's necessary, even before sending to us.
We'll need you to send us the original radiographs or digital images, along with the
Transmittal Form. It's really important that you get us these promptly, so that we
have a chance to review the images and make sure they're okay, and if a repeat is
required we can get the patient back in the clinic as soon as possible.
Also, please note that we are relying on you to report any indications of a possible
Serious Adverse Event. So if the subject comes in and reports what you think could be
an Adverse or Serious event, please talk to your investigator and notify Pfizer
immediately—definitely within 24 hours.

So what is an Adverse Event?
It could be something simple, like the patient indicating that they're not feeling well;
but it could also be indication of abnormal test findings, or other clinically significant
symptoms or signs; changes in their physical examination; or just noticing progression
or worsening of the underlying disease.

So, radiographs are going to be acquired at several time points: at the baseline,
Month 3 visit (note this is for non-responders only), everybody is going to have
radiographs at the Month 6, Month 12, and at the Month 24/End of Study visit.

Let's talk for a minute about how the images are going to be analyzed.
The radiographs are going to be scored for disease and disease progression, using the
Modified Sharp-van de Heijde Scoring Method.

On the left, you see an image of the hand, with the 15 locations in the hand and wrist
at which erosion is going to be scored.
On the right, you see a radiograph of the foot, with the six locations where erosion is
scored. Notice that in the foot, we are only scoring the forefoot, and the ankle,
while depicted on the radiograph, isn't scored. This has implications for how the
contrast should be set, and we'll talk about that in a bit.

The other feature that we score is joint-space narrowing.
Joint-space narrowing is scored in the 15 locations shown at the left, in the hand and
wrist, and in the same six joints of the forefoot.
It is really important to note that the way the radiologists are going to be reading
these, they are going to be looking at a series of radiographs altogether, not knowing
which time point is which, while trying to assess change.
What this means is that any variation in the image acquisition—say a change in beam
angle, for example—could mask as artificial change—let's say joint-space narrowing,
or even erosion, an erosion getting bigger or smaller. So it's going to be really
important to ensure that the radiographs are acquired consistently.

Да поговорим малко за това как ще бъдат анализирани изображенията.
Рентгенографиите ще се оценяват за заболяване и прогресия на заболяването, използвайки модифицирания метод за отчитане на резултатите на Sharp-van der Heijde.

Наистина е важно да се отбележи начинът, по който рентгенолозите ще разчитат тези снимки, те ще разглеждат поредица от снимки наведнъж без да знаят коя по кое време е правена, като в същото време ще се опитат да преценят промяната.
Което ще рече, че всяко отклонение при изпълнение на изображението—например, промяна в ъгъла на лъча—може да доведе до маскирана промяна—да кажем, стесняване на ставната междина, или дори ерозия, ерозия, която става по-голяма или по-малка.
