We know it is sometimes hard to take your medicines exactly as prescribed. There are many reasons but for example people may forget to take their medicines, decide to stop taking a medicine because they feel unwell when they take it, forget to order a repeat prescription, feel it is not necessary to take anymore, or decide to increase or decrease the dose they take. These are only examples.
We would like you to think about each one of your heart medicines, specifically the ones listed below. Please indicate by ticking () if you take one or more of these heart medicines. For each medicine you take we would like you to estimate in the past MONTH how often did you take your medicine as the doctor prescribed?

ہم جانتے ہیں کہ بعض اوقات آپ کے لئے اپنی ادویات کا استعمال کرنا بالکل اسی طرح جیسا کہ تجویز کیا گیا ہو مشکل ہوتا ہے۔ اسکی کئی وجوہات ہیں لیکن مثال کے طور پر لوگ اپنی دوا کھانا بھول جاتے ہیں، کسی دوا کو چھوڑ دینے کا فیصلہ کرتے ہیں کیونکہ وہ اسے لینے کے دوران بہتری محسوس نہیں کرتے، دوبارہ نسخہ کو طلب کرنا بھول جاتے ہیں، وہ سمجھتے ہیں کہ اسے مزید لینا ضروری نہیں ہے یا جو خوراک وہ لے رہے ہیں اس میں اضافہ یا کمی کا فیصلہ کرتے ہیں۔ یہ صرف مثالیں ہیں۔
