La LVH hipertensiva se desarrolla en respuesta a rutas de estrés activadas por la sobrecarga mecánica impuesta por la hipertensión sistémica sobre el ventrículo izquierdo. Se ha propuesto que algunos de estas rutas implican la participación de ligandos dependientes de gp 130.
La cardiotrofina-1 (CT-1), una citoquina que induce hipertrofia de cardiomiocitos y estimula a los fibroblastos cardíacos, es un miembro de 201 aminoácidos de la superfamilia de la interleuquina -6, que se une a la glicoproteína 130 (gp 130) y al receptor del factor inhibidor de leucemia. The determining criterion for diagnosing hypertensive heart disease is the presence of left ventricle hypertrophy (LVH) in the absence of any cause other than high blood pressure. Two factors are currently accepted as contributing to LVH in hypertension: a) cardiac myocyte growth and b) exaggerated deposits of collagen fibres leading to myocardial fibrosis. Hypertensive LVH develops in response to stress pathways activated by the mechanical overload caused on the left ventricle by systemic hypertension. It has been proposed that some of these pathways imply the participation of ligands dependent on glycoprotein 130 (gp 130).
Cardiotrophin-1 (CT-1) is a cytokine that induces cardiac myocyte hypertrophy and stimulates cardiac fibroblasts. It is a member of 201 amino acids belonging to the interleukin-6 superfamily, and links to gp 130 and the leukaemia inhibitory factor receptor. At cardiac level, CT-1 is synthesised and secreted by both cardiac myocytes and non-cardiac-myocyte cells. Although some in vitro findings suggest that CT-1 has properties promoting cardiac myocyte growth and survival, various in vivo studies have shown that systematic CT-1 administration is associated with cardiac hypertrophy. Ventricular CT-1 gene expression is known to increase in rats with spontaneous hypertension in the first stage of LVH and to remain elevated after hypertrophy develops. Heart levels of CT-1 messenger RNA and the protein itself are also higher in hypertensive Dahl salt-sensitive rats with LVH compared with normotensive Dahl salt-resistant rats. CT-1, its receptors and signalling pathways have been found in genes and in cardiac fibroblast proteins. CT-1 has also been shown to stimulate type 1 collagen proliferation and synthesis directly in this type of cells. These findings indicate that CT-1 may contribute to the development of hypertensive heart disease, both by inducing cardiac myocyte hypertrophy as well as by promoting myocardial fibrosis.
