A Multicenter, Open-Label, Randomized, Phase II/III Study to Evaluate the Safety and Efficacy of Medication A in Combination with
Medication B and Medication C in comparison with Medication B and Medication C alone in the Treatment against Anaplastic Thyroid Carcinoma

Objectives:
Primary Objective:
• To compare the antineoplastic efficacy of Medication A Medication B Medication C with Medication B Medication C against ATC by measuring overall survival
Secondary Objectives:
• To evaluate the safety and tolerability of the triple combination of Medication A Medication B Medication C
• To assess specified objective events:
tracheostomies, PEG tube placements, and
weight loss
Exploratory Objectives:
• To assess the potential impact on survival by
stratifying for thyroid / local /regional metastases versus distant metastases
as well as for tumor size (≤ 6 cm vs. > 6 cm)
• To assess whether changes in specific uptake values (SUV) on FDG PET Imaging can predict overall survival
• To assess exposure-response relationships of Medication A
• To assess the effects of various covariates (e.g., age, gender, race), and the inter- and intra-subject variability of Medication A and its metabolites
Study Design and Duration
This is a multicenter, open-label, randomized, two-armed Phase II/III study of approximately 180 patients randomized 2:1 to arm 1 and arm 2 respectively. Arm 1 will be comprised of 120 patients being treated with intravenous Medication A administered at 60 mg/m2 on Days 1, 8, and 15 of each 3 week cycle followed by Medication B (3 hour infusion) and Medication C (1 hour infusion) on Day 2 of each 21-day cycle for up to 6 cycles. This triplet chemotherapy will be followed by a maintenance dose of Medication A on Days 1 and 8, every 21 days until disease progression. Arm 2 will be comprised of 60 patients being treated with Medication B (3 hour infusion) and Medication C (1 hour infusion) on Day 1 of each 21-day cycle for up to 6 cycles.
All patients will be followed for survival until reaching a total of 125 deaths from the study patients. The study will consist of a screening visit within 14 days before study drug administration, 21-day combination treatment cycles (up to 6 cycles), and an end of treatment visit. If the patient is randomized to arm 1, a maintanence program of Medication A monotherapy consisting of 21-day cycles will follow until disease progression, as well as, an end of maintenance therapy visit. All patients will be followed monthly once off study via telephone until death. Any serious study drug related toxicities will be followed for 30 days after the last dose of the study treatment until resolution, return to baseline or Grade 1 or until the events are deemed irreversible. Any non-serious study drug related toxicities that still exist 30 days after the last dose of the combination therapy will be deemed ongoing and follow-up will be discontinued. (Refer to section 10.1 on definitions of serious and non-serious adverse events.)
It is estimated that overall patient accrual will occur over approximately 18 months.

The importance of guilt in the aetiology of neuroses and psychoses is well known. In my opinion,
however, the question of guilt, of its origin, nature, and different ways of participation in the individual's mental development, is still one of the problems in the field of psycho-analytic research that has not yet been fully elucidated.
In some psycho-analytic circles different groups of therapists have developed varying trends of thought which differ mainly-among other things-in the way they handle guilt in their respective techniques. One or these trends centres interpretation around the necessity of liberating the patient from a guilt which, according
to them, is of a negative and pathological nature, to which he is bound in a masochistic manner. Other
analysts, on the other hand, seem to follow an utterly opposed theoretical-technical criterion in regard to this problem. For them, the core of any neurotic conflict is, in fact, centred on the denial of guilt experienced by the individual on account of aggressive fantasies directed towards the objects. In my opinion this controversy results from the mistake of dealing with two different kinds of guilt under the assumption that they are one
and the same.
One of the classic starting-points established by Freud is that guilt, proceeding from tensions between the ego and the superego, appears as a need for punishment. Freud (191 3), when studying the oedipal conflict in primitive societies, shows how out of the sense of guilt sprang the two main taboos of totemism: parricide and incest. After the sons had killed the father of the horde, remorse and guilt appeared. Totemism, according to Freud, is the earliest form of religion in the history of mankind . Under the influence of guilt the ego submits itself to the demands of the superego, fearing to lose its love and protection.
On the other hand, Freud (1916) showed that guilt arises to a much greater degree from unconscious
fantasies than from actual deeds. The actual crime is frequently not the real motive for guilt but rather its consequence.
According to Freud's later work (1930) the guilt belongs to the realm of aggressive instincts, aggression being turned towards the ego as a punishing force, acting through the superego.
Klein (1935) also believes that guilt is linked with the emergence of the superego, though she places it at an earlier period, that is, during the stage in which the infant's sadism plays a dominant role. She points out that the first feelings of guilt arise from the child's oral-sadistic desires to devour the mother, especially the breast (Abraham).

חשיבות רגשות אשמה באטיולוגיה של נוירוזות ופסיכוזות מוכרת היטב. לאומת זאת, שאלת האשמה, מקורותיה, טבעה והאופנים השונים להשתתפותה בהתפתחות הנפשית של הפרט, עדיין מהווה לדעתי את אחת הבעיות בתחום המחקר הפסיכואנליטי שטרם הובהרה במלואה.
בחוגים פסיכואנליטיים מסוימים, קבוצות שונות של מטפלים פיתחו מגוון מגמות מחשבה, הנבדלות, בין יתר המאפיינים, בעיקר באופן בו הם מתייחסים לאשמה בשיטותיהם. אחת המגמות האלה מרכזת את הפירוש (אינטרפרטציה) סביב הצורך לשחרר את המטופל מהאשמה, אשר לדעתם, בעלת טבע שלילי ופתולוגי אליו הוא נקשר באופן מזוכיסטי. מצד שני, נראה כי אנליסטים אחרים מצדדים בקריטריון תיאורטי-טכני מנוגד לחלוטין בהקשר לנושא זה. עבורם, תמציתו של כל קונפליקט נוירוטי למעשה מרוכזת בהכחשת האשמה, אותה חווה הפרט בשל פנטזיות אגרסיביות המכוונות כלפי אובייקטים. לדעתי ניגוד זה נובע מעיסוק מוטעה בשני סוגים של אשמה תחת ההנחה שהם סוג אחד וזהים.
