Two hypotheses will be tested using a step down approach for the primary endpoint. Source text - English Surface Heating of Invasive Transducers The average and peak radiated acoustic powers of all [Company] transducers are limited by detection circuitry, to ensure that the surface heating of the transducer array is less than 43ºC. To limit the average acoustic power output, the maximum pulse repetition frequency (PRF) is tested in hardware by comparing a PRF counter to a preset value. When the maximum allowable PRF is exceeded, the ultrasound system displays an error message, and disables the acoustic output. The preset maximum PRF is chosen to limit the transducer case temperature to less than 43ºC when the transducer is touching a patient. The peak acoustic power is constrained by the maximum voltage applied to the transducer -array elements. In hardware, peak power is monitored by a watchdog device that measures instantaneous power and compares it to two thresholds. The first threshold is a nonprogrammable four-watt limit. Source text - English OVERVIEW OF CLINICAL TRIAL DESIGN This will be a Phase 3A prospective, multi-country, multi-center, open-label, randomized, controlled clinical trial of approximately 668 subjects with type 2 diabetes, comparing those who are treated with subcutaneous basal insulin in combination with inhaled prandial T/I versus a subcutaneous premix of intermediate acting and rapid acting insulin (ie, BiPhasic Rapid Acting Insulin 70/30, herein referred to as “BPR 70/30”). The clinical trial will have a duration of approximately 52 weeks of treatment and a 4-week follow-up. The clinical trial consists of 13 visits: Screening Visit (Visit 1), a Randomization Visit (Visit 2), a baseline visit (Visit 3), and 4 titration visits (Visits 4, 5, 6, and 7) to adjust insulin therapy. Between Visit 7 and Visit 8, there will be 3 telephone “visits” to titrate dose if necessary. Five additional visits will occur during the treatment period at regular intervals (Visits 8, 9, 10, 11, and 12). Visits 3, 8, 9, 10, and 12 will include clinical trial assessments and procedures. Visit 11 will be a drug re-supply visit only. Visit 13 (Week 56) will be a 4-week follow up that will occur after the end of treatment and will include PFT tests. Source text - English 1 Using sterile technique, remove the instrument from the package. To avoid damage, do not flip the instrument into the sterile field. 2 The trocar obturator and sleeve are packaged unassembled. To assemble, remove the protective tip covering from the obturator and trocar sleeve and discard. Assemble the trocar by inserting the obturator into the trocar sleeve until they lock securely together. Note: The trocar is packaged with the stopcock in the open position. Close the stopcock before use. The stopcock is in the closed position when the stopcock lever is parallel to the sleeve. 3 Connect the appropriately sized 0° endoscope to the light supply and monitor as directed in the manufacturer’s instructions. Verify proper connection of the endoscope and ensure the clarity of the picture on the monitor. 4 Insert the endoscope into the opening at the proximal end of the obturator until it reaches the distal tip of the obturator (Illustration 2). 5 Rotate the endoscope as desired. Secure the endoscope in the obturator using the scope locking cam (Illustration 3). 6 To provide a clear image on the monitor, insert the endoscope into the obturator, touch the tip of the optical element to a convenient soft surface, and focus the camera. 7 Create an incision using standard surgical procedure which allows the trocar to be introduced. Note: An inadequate incision may cause increased resistance to insertion, increasing the required penetration force, and possibly resulting in a loss of control during entry. 8 Introduce the obturator through the skin incision using a 30º to 90º rotating motion. Apply light and continuous but controlled downward pressure on the obturator (Illustration 4). View the penetration of the obturator tip through the individual tissue planes by using the endoscope and video camera. The individual tissue planes may be seen as the obturator tip advances. 9 When the trocar is in the abdominal or thoracic cavity, press the locking buttons to remove the obturator and endoscope, leaving the sleeve in place. Release the scope locking cam and remove the endoscope from the obturator. The internal seal in the sleeve automatically closes as the obturator is withdrawn. The seal system maintains insufflation in the absence of an instrument in the sleeve (Illustrations 5 and 6). 10 To insufflate, attach a gas line to the stopcock on the trocar sleeve and open the stopcock. Source text - English MANUALLY CORRECTING THE AUTOMATIC SEGMENTATION The Liver segmentation tools allow you to correct the automatic segmentation renderings, generated when the study was loaded into the application, and to resegment the liver for improved image renderings. Attract To Curve - allows you to correct liver tissue. Click the tool and put points along the border of the liver where the segmentation was not accurate enough. A spline is created connecting the points. End the line by double clicking, or by the keyboard key. If needed, you can edit the line after it is finalized. This tool is also used to correct either over or under segmentation. Finish the correction by clicking on the checkmark at the right side of the Attract To Curve tool. The correction works on multiple slices, not just on the slice where tool was applied. Inject dye - works as in CT Viewer. The added tissue is marked with blue overlay on the slab images. The injection rate can be selected from the Rate dropdown by clicking the arrow and selecting the desired rate. Click the checkmark icon to apply the correction. Eraser - similar to CT Viewer, creates an erasing sphere. The eraser works directly on the liver tissue. If injection was applied, the eraser works on the injection blue overlay and not on the liver tissues. Select either the Large, Medium, or Small eraser from the eraser dropdown. Expand - allows you to increase the edges of the liver tissue. Each click expands the edge by a one-voxel increment. Erode - allows you to decrease the edges of the liver tissue. Each click reduces the edge by a one-voxel increment. При каждом щелчке граница расширяется на один воксель. Erode (Размытие) - позволяет уменьшить границы ткани печени. При каждом щелчке граница уменьшается на один воксель.
