FEATURE FOR MICROCALCIFICATION DETECTION
The features based on the shape are evaluated within an area of interest (AOI), made of a square box of 17 × 17 pixels centered on the centroid of the microcalcification:
• Compactness: evaluated as the ratio between the area of the microcalcification and the square of the perimeter of its border. According to its definition, the higher the compactness, the more similar to a circle the shape;
• Roughness: it is a measure of the irregularity of the border. It is defined as the standard deviation of the square distance of the points of the border from the center of the microcalcification. The higher the roughness, the more irregular the border of the microcalcification;
• Border gradient strength: it is an estimate of the intensity gradient measured on the points of the border of the microcalcification. The higher this parameter, the more contrasted the border of the microcalcification;
• Local contrast: it is a measure of the average difference between the intensity of the points belonging to the microcalcification and the intensity of the points belonging to the background. The higher this parameter, the more distinguishable the microcalcification from the surrounding tissue.
Features based on the texture:
• Energy in the AOI: the average square intensity in the AOI;
• Energy in the background: the average square intensity in the background;
• Average intensity in the AOI;
• Standard deviation of the intensity in the AOI;
• Entropy of the first-order histogram: a measure of the uniformity of the distribution of the gray levels in the AOI. The higher this parameter, the more uniform the distribution of gray levels in the AOI;
• Energy of the second-order histogram: the average square value of the co-occurrence matrix evaluated on the AOI;
• Contrast of the second-order histogram: a measure of the distribution of the difference among the gray levels exhibited by the points in the AOI
• Entropy of the second-order histogram: a measure of the uniformity of the distribution of the values in the co-occurrence matrix.
FEATURE FOR CLUSTER DETECTION
• Mass density of the cluster;
• Average mass of the microcalcifications: a measure of the compactness of the microcalcifications;
• Average distance between microcalcifications and the center of mass of the cluster: a measure of the roughness of the shape of the cluster;
• Standard deviation of the masses of the microcalcifications: a measure of the variation of the compactness;
• Standard deviation of distance between microcalcifications and center of mass: a measure of the variation of the roughness.

I parametri basati sulla forma vengono valutati all’interno di un’area di interesse (AOI, area of interest), costituita da una cella di 17 x 17 pixels il cui centro geometrico si trova al centro della microcalcificazione:
• Compattezza: valutata come rapporto fra area della microcalcificazione e quadrato del perimetro del suo contorno. In base alla sua definizione, maggiore è la compattezza, più la forma si avvicina a un cerchio;
• Irregolarità del margine: è una misura dell’irregolarità del contorno. È definita come la deviazione standard dei quadrati delle distanze dei punti del margine dal centro della microcalcificazione. Più alta è questa misura, maggiormente irregolare sarà il margine della microcalcificazione;
• Forza del gradiente sul margine: è una stima del gradiente di intensità misurato sui punti del margine della microcalcificazione. Più alto è questo parametro, maggior contrasto si osserva sul margine della microcalcificazione;
• Contrasto locale: è una misura della differenza media tra l’intensità dei punti che appartengono alla microcalcificazione e l’intensità dei punti che appartengono allo sfondo. Più alto è questo parametro, meglio si distingue la microcalcificazione dal tessuto circostante.
Parametri basati sulla tessitura
• Energia nella AOI: la media del quadrato dell’intensità nella AOI;
• Energia dello sfondo: la media del quadrato dell’intensità dello sfondo;
• Intensità media nella AOI;
• Deviazione standard dell’intensità nella AOI;
• Entropia dell’istogramma di primo ordine: una misura dell’uniformità della distribuzione dei livelli di grigio nella AOI. Più alto è questo parametro, più uniforme è la distribuzione dei livelli di grigio nella AOI;
• Energia dell’istogramma di secondo ordine: la media del quadrato dei valori della matrice di co-occorrenza valutata sulla AOI;
• Contrasto dell’istogramma di secondo ordine: una misura della distribuzione della differenza fra i livelli di grigio dei punti nella AOI;
• Entropia dell’istogramma di secondo ordine: una misura dell’uniformità della distribuzione dei valori nella matrice di co-occorrenza.
PARAMETRI PER L’INDIVIDUAZIONE DEL CLUSTER
• Densità della massa del cluster;
• Massa media delle microcalcificazioni: una misura della compattezza delle microcalcificazioni;
• Distanza media tra le microcalcificazioni e il centro di massa del cluster: una misura dell’irregolarità della forma del cluster;

Diagnostic criteria – how to interpret NMR images for breast cancer screening
To reaach a high sensitivity and a reasonable specificity it is necessary to assess not only the change of a signal intensity in time but simultaneously also other criteria – the shape of a lesion, margins, structure (pattern – homogeneity of enhancement) and temporal distribution of enhancement (contrast kinetics).
Analogically, like in mammography, the shape of enhancement may be round, oval, polygonal, linear, branching, spiculated. The signs typical for malignity are shown in red. Margins well or ill-defined , structure homogenous, inhomogenous, septated, ring enhancement. Temporal contrast distribution is centrifugal. unchanging or centripetal.
Change of the signal intensity with respect to time is described by a signal-time curve. The curve has an initial phase, up to 3 minutes, and then a continuous phase. The initial signal increase may be slight (by less than 50% of the pre-contrast signal), moderate (an increase by 50 – 100%) and strong (an enhancement over 100%). The threshold is usually about 100% of signal intensity during the 3rd minute (it is good compromise – we get high sensitivity and not so bad specificity). In the next phase the signal either grows (continuous increase), fundamentally remains the same (Plateau) or decreases (wash-out).
Typical for malignity is branching or spiculated shape, indistinct margins, centripetal ring enhancement with initial enhancement over 100% with subsequent wash-out. Szabo from Sweden and his colleagues believe so called Time to Peak to be an important criteria as well.

Per raggiungere un’alta sensibilità e una giusta specificità è necessario determinare non soltanto il cambiamento nel tempo dell’intensità del segnale, ma contemporaneamente anche altri criteri – la forma di una lesione, i margini, la struttura (caratteristiche – omogeneità dell’enhancement) e la distribuzione temporale dell’enhancement (cinetiche di contrasto).
Dal punto di vista analogico, come nella mammografia, la forma dell’enhancement può essere rotonda, ovale, poligonale, lineare, ramificata, a spicchi. I tipici segni di lesione maligna sono mostrati in rosso. I margini ben definiti o patologici, le strutture omogenee, disomogenee, settate, con enhancement ad anello. La distribuzione temporale del contrasto è centrifuga, invariabile o centripeta.
Il cambiamento dell’intensità di segnale rispetto al tempo è descritta da una curva segnale-tempo. La curva ha una fase iniziale, fino ai 3 minuti, e quindi una fase continua. L’aumento del segnale iniziale può essere leggero (inferiore al 50% del segnale pre-contrasto), moderato (un aumento del 50 – 100%) o forte (un aumento superiore al 100%). La soglia è generalmente pari a circa il 100% dell’intensità di segnale durante il terzo minuto (è un buon compromesso – otteniamo un’alta sensibilità e una specificità non così malvagia). Nella fase successiva, o il segnale cresce (aumento continuo), o rimane fondamentalmente uguale (plateau) o diminuisce (sospensione).
Tipica della lesione maligna è la forma ramificata o a spicchi, margini indistinti, enhancement centripeto ad anello con enhancement iniziale superiore al 100% e successiva sospensione. Szabo dalla Svezia e i suoi collaboratori ritengono che anche il cosiddetto “tempo al picco” (Time to Peak) sia un criterio importante.
