We have become aware of potential issues with some of our products.
Issues • Sample slides that follow the analysis of manufactured controls may exhibit carryover of cellular subcomponents of the controls which may be mis-identified as an abnormal human cell or an unidentifiable cell.
• Studies have been performed which included analysis of a human blood sample with a WBC count approximately 90 X 103/ul with a subsequent slide made from a red cell pool. While physiological variations can occur at the specimen level, our testing has determined within 95% confidence limits, zero to one cell may be carried over to the working area of a subsequent patient sample slide at a WBC level of 90 x 103/ul (description below).
The Clinical and Laboratory Standards Institute (CLSI) (formerly known as NCCLS] provides the description of the working area for the microscopic examination of the blood film in their document H20-A2E. The examination extends from “the area where approximately 50% of the erythrocytes overlap to the region where erythrocytes show a tendency to linear orientation, which, in even thinner parts of the film, causes separate streaks known as the ‘feather edge’.”
Based on the carryover studies, the following action should be taken when analyzing controls and preparing patient sample slides which follow samples with elevated WBC counts.
Action for patient sample processing 1. Based on the elevated WBC carryover performance described in this letter, each laboratory must develop a protocol for repeat preparation of patient sample slides for situations where carryover of cells from a previous patient sample may have occurred.

Problèmes • Les lames spécimen faisant suite à l’analyse de contrôles manufacturés peuvent présenter une contamination des sous-composants cellulaires des témoins et être pris à tort pour une cellule humaine anormale ou non identifiée.
• Des études ont été réalisées avec l’analyse d’un échantillon de sang humain présentant une NGB d’environ 90 X 103/ul suivi d’une lame préparée à partir d’un pool d’érythrocytes. Malgré des variations physiologiques possibles au niveau du spécimen, notre test a montré, avec une limite de confiance de 95%, que zéro à une cellule provenant d’une lame spécimen au taux NGB 90 x 103/ul, peut contaminer la zone de travail de la lame spécimen ultérieure (cf. description ci-dessous).
Dans son document H20-A2E, le CLSI (Clinical and Laboratory Standards Institute autrefois dénommé le NCCLS) décrit la zone de travail pour l’examen microscopique du frottis sanguin. Compte tenu des études de contamination inter-échantillons, les mesures suivantes devraient être prises pour l’analyse des témoins et la préparation des lames spécimen de patient suivant des échantillons à NGB élevée.
Mesures à prendre pour le traitement des prélèvements 1. Compte tenu des problèmes de contamination associée à une NGB élevée décrits dans cette lettre, tous les laboratoires doivent développer un protocole pour la préparation répétée de lames spécimen dans le cas où les cellules d’un premier échantillon auraient interféré avec celles de l’échantillon suivant.
