Pada hari ini, Senin tanggal Dua Puluh Lima bulan April tahun Dua Ribu Sebelas (25-04-2011), kami yang bertandatangan di bawah ini :
I. ------
: Kepala Dinas Kesehatan Provinsi Jawa Barat Selaku Wakil Ketua Komisi Penanggulangan AIDS (KPA) Provinsi Jawa Barat, berkedudukan di Bandung, Jalan Pasteur Nomor 25, berdasarkan Surat Kuasa Khusus Gubernur Jawa Barat Nomor ……./…../…………. tanggal …………………… 2011, dalam hal ini bertindak untuk dan atas nama Pemerintah Provinsi Jawa Barat, selanjutnya disebut PIHAK KESATU.
II. ------- : Capacity Development Adviser Indonesian HIV Cooperation Program for Indonesia (HCPI), berkedudukan di Jakarta Pusat, Wisma Kosgoro Lantai 3, Jalan M.H. Thamrin Nomor 53, berdasarkan Surat Sekretaris Negara Nomor B - 7710/Setneg/Setmen/KTLN/ 04/2008 perihal Persetujuan penugasan tenaga-tenaga ahli luar negeri dalam rangka kerjasama teknik dengan Pemerintah Australia, dalam hal ini bertindak untuk dan atas nama HIV Cooperation Program for Indonesia, selanjutnya disebut PIHAK KEDUA.
PIHAK KESATU dan PIHAK KEDUA selanjutnya secara bersama-sama dalam Perjanjian Kerjasama ini, disebut PARA PIHAK.
Terlebih dahulu PARA PIHAK menjelaskan hal-hal sebagai berikut :
1. Provinsi Jawa Barat merupakan salah satu wilayah utama penyebaran virus HIV dan AIDS di Indonesia. Untuk menanggulangi hal tersebut, PIHAK KESATU menyelenggarakan koordinasi pelaksanaan kebijakan nasional penanggulangan HIV dan AIDS di Jawa Barat berdasarkan Keputusan Gubernur Jawa Barat Nomor 443/Kep.688-Yansos/2009, dalam rangka perumusan dan penetapan kebijakan, penyelenggaraan koordinasi, pemantauan dan evaluasi, pengkoordinasian serta pelaksanaan kerjasama regional dan penyebarluasan informasi mengenai upaya penanggulangan HIV dan AIDS.
2. PIHAK KESATU telah memiliki 4 (empat) orang staf dalam kesekretariatan yang didukung oleh Komisi Penanggulangan AIDS Nasional melalui bantuan Global Fund for AIDS, Tuberculoses, and Malaria (GF-ATM) yaitu Pengelola Program, Pengelola Monitoring dan Evaluasi, Pengelola Keuangan, dan Pengelola Administrasi dan 1 (satu) orang staf yang didukung oleh PIHAK KEDUA yaitu Media Relation Officer. Namun dengan luas dan kompleksnya permasalahan HIV dan AIDS di Jawa Barat maka dibutuhkan penambahan sumberdaya manusia untuk penguatan kesekretariatan.
3. PIHAK KEDUA mempunyai pengalaman dan staf yang berkompeten untuk meningkatkan kapasitas kesekretariatan yang dapat meningkatkan kemampuan kinerja PIHAK KESATU agar dapat mengoptimalkan upaya pencegahan, pengendalian dan penanggulangan HIV dan AIDS melalui langkah-langkah strategis untuk menjaga kelangsungan penanggulangan HIV dan AIDS dan menghindari dampak yang lebih besar di bidang kesehatan, sosial, politik dan ekonomi.
Berdasarkan hal-hal tersebut, PARA PIHAK sepakat untuk melakukan Perjanjian Kerjasama tentang Dukungan Kesekretariatan Komisi Penanggulangan AIDS (KPA) Provinsi Jawa Barat (selanjutnya disebut Perjanjian Kerjasama), dengan ketentuan dan syarat-syarat sebagai berikut:
MAKSUD DAN TUJUAN
Pasal 1
(1) Maksud Perjanjian Kerjasama ini adalah mewujudkan optimalisasi fungsi kesekretariatan Komisi Penanggulangan AIDS (KPA) Provinsi Jawa Barat guna mendukung upaya pencegahan dan penanggulangan HIV dan AIDS di jawa Barat.
(2) Tujuan Perjanjian Kerjasama ini adalah dalam rangka meningkatkan pencegahan dan penanggulangan HIV dan AIDS.
OBJEK
Pasal 2
Objek Perjanjian Kerjasama ini adalah :
a. dukungan kesekretariatan Komisi Penanggulangan AIDS Provinsi (KPA) Provinsi Jawa Barat; dan
b. pengelolaan program penanggulangan HIV dan AIDS di Jawa Barat.
RUANG LINGKUP
Pasal 3
Ruang lingkup Perjanjian Kerjasama ini meliputi :
a. dukungan Sumber Daya Manusia dari PIHAK KEDUA yaitu Provincial Coordinator, Provincial Technical Officers, Admin and Finance Assistance, Office Assistance, dan Driver ke dalam Struktur Sekretariat Komisi Penanggulangan AIDS (KPA) Provinsi Jawa Barat;
b. pengelolaan kerjasama dan kemitraan;
c. pengelolaan dan program hubungan media;
d. kegiatan advokasi; dan
e. pengelolaan program harm reduction.
PENUTUP
Pasal 16
Hal-hal yang belum dan/atau belum cukup diatur dalam Perjanjian Kerjasama ini akan diatur oleh PARA PIHAK, berdasarkan kesepakatan yang dituangkan dalam Perjanjian Tambahan (Addendum), sebagai bagian yang tidak terpisahkan dari Perjanjian Kerjasama ini.
Demikian Perjanjian Kerjasama ini dibuat dan ditandatangani oleh PARA PIHAK di Bandung pada hari, tanggal, bulan dan tahun tersebut di atas dalam rangkap 3 (tiga) bermeterai cukup, masing-masing mempunyai kekuatan hukum yang sama.

Today, Monday twenty-fifth of April two thousand and eleven (25-04-2011), we, who undersign herein :
I. ---------
: Head of Health Office of West Java as the Vice Chairman of Provincial AIDS Commission (PAC) of West Java, residing in Bandung, Jalan Pasteur Nomor 25, based on the Special Power of Attorney of the Governor of West Java Number ...../...../........... dated .................................... 2011, in this matter acting for and on behalf of the West Java Provincial Government, hereinafter referred as FIRST PARTY.
II. -------- : Capacity Development Adviser Indonesian HIV Cooperation Program for Indonesia (HCPI), residing in Jakarta Pusat, Wisma Kosgoro Lantai 3, Jalan M.H. Thamrin Nomor 53, based on the Letter of the State Secretary Number B – 7710/Setneg/Setmen/KTLN/ 04/2008 regarding approval of the assignment of foreign experts in the technical cooperation with the Australian Government, in this matter acting for and on behalf of HIV Cooperation Program for Indonesia, hereinafter referred as SECOND PARTY.
The FIRST PARTY and the SECOND PARTY in this Cooperation Agreement hereinafter are collectively referred as PARTIES.
First of all, PARTIES shall describe the followings:
1. West Java is one of the key areas of the spread of HIV and AIDS virus in Indonesia. In order to control the situation, FIRST PARTY conducts coordination of HIV and AIDS control national policy implementation in West Java based on the Decree of the Governor of West Java Number 443/Kep.688-Yansos/2009, in formulating and setting up policy, conducting coordination, monitoring and evaluation, regional cooperation coordination and implementation and information dissemination on HIV and AIDS control.
2. FIRST PARTY has hired 4 (four) staffs in the secretariat that are supported by National AIDS Commission through funding from Global Fund for AIDS, Tuberculosis and Malaria (GF-ATM) entitled as Program Manager, Monitoring and Evaluation Manager, Financial Manager and Administration Manager and 1 (one) staff that is supported by SECOND PARTY entitled as Media Relation Officer. Considering the HIV and AIDS problem range and complexity in West Java, human resources addition is required to reinforce the secretariat.
3. SECOND PARTY has expertise and competent staff to improve secretariat capacity in promoting the performance of FIRST PARTY in order to enhance the effort of HIV and AIDS prevention and control through strategic actions by maintaining the sustainability of the effort and preventing larger impacts toward health, social, politic and economic condition.
Based on the aforementioned points, the PARTIES agree to conduct Cooperation Agreement on Secretarial Support to West Java Provincial AIDS Commission (PAC) (hereinafter referred to Cooperation Agreement), hereby is under the following terms and conditions:
PURPOSE AND AIM
Article 1
(1) The purpose of this Cooperation Agreement is to achieve optimum secretarial function of the West Java Provincial AIDS Commission (PAC) to promote the effort of HIV and AIDS prevention and control in West Java.
(2) The aim of the Cooperation Agreement is enhancing the HIV and AIDS prevention and control.
OBJECTS
Article 2
The objects of this Cooperation Agreement are as follows:
a. support to Provincial AIDS Commission (PAC) of West Java secretariat; and
b. HIV and AIDS control program management in West Java.
SCOPE
Article 3
The scope of this Cooperation Agreement covers the followings:
a. Human Resources support from SECOND PARTY in the position of Provincial Coordinator, Provincial Technical Officers, Admin and Finance Assistance, Office Assistance, and Driver as part of the Secretarial Structure of West Java Provincial AIDS Commission (PAC);
b. cooperation and partnership management;
c. media relation management and program;
d. advocacy program; and
e. harm reduction program management.
CLOSING
Article 16
Any issue which has not and/or not adequately stated herein the Cooperation Agreement shall be discussed by the PARTIES in accordance to the agreement set forth therein Addendum as one of the forming parts of this Cooperation Agreement.
In witness whereof the PARTIES hereto have made and sign this Cooperation Agreement in Bandung, on the date first set forth above in 3 (three) adequately duty stamped originals occupying equal legal capacity.

Part One – Ghost Wife
She was his first wife, but at the moment when he first saw her she was a seventeen-year-old girl named Arlyn Singer who stood out on the front porch on an evening that seemed suspended in time. Arlyn’s father had just died and the funeral dinner had ended only hours earlier. It was a somber gathering: a dozen neighbors seared around the heavy mahogany dining-room table no one had used for over a decade. Now there were pans of macaroni and cheese and a red velvet cake and a huge platter of fruit, food enough to last a month if Arlie had had an appetite.
Arlyn’s father had been a ferryboat captain, the center of her world, especially in his last years; the captain had burned brighter in the grasp of his illness, a shining star in the dark. A usually silent man, he began to tell stories. There were tales of rocks that appeared in the dark, of a mysterious reefs whose only purpose seemed to be to sink ferries, of the drowned men he’d known who had never come back. With a red crayon, he drew charts of stars that could lead a lost man home. He told of a tribe who lived on the other side of the water, in far-off Connecticut, who could sprout wings in the face of disaster. They looked like normal people until the ship went down, or the fire raged, and then they suddenly revealed themselves. Only then did they manage their escape.
On his night table there was a collection of stones the captain said he had swallowed when he was a young man; he’d gone down with a ship and had been the lone survivor. One minute he’d been standing on deck, and the next, he’d been above it all, in the sky. He’d fallen hard and fast into the surf of Connecticut, with a mouth and belly full of stones.
When the doctor came to tell the captain there was no hope, they had a drink together and instead of ice the captain put a stone in the cups of whiskey.
It will bring you good luck, he’d told the doctor. All I want is for my daughter to be happy. That’s all the luck I need.
Arlyn had sobbed at his bedside and begged her father not to leave her, but that was not an option or a choice. The last advice the captain had given her, while his voice still held out, was that the future was an unknown and unexpected country, and that Arlyn should be prepared for almost anything. She had been grief stricken as her father lay dying but now she felt weightless, the way people do when they’re no longer sure they have a reason to be connected to this world. The slightest breeze could have carried her away, in to the night sky, across the universe.
Arlyn held on to the porch banister and leaned out over the azaleas. Red and pink flowers, filled with buds. Arlyn was an optimist, despite her current situation. She was young enough not to see a glass as half empty or half full, but as beautiful object into which anything might be poured. She whispered a bargain, as though her whispering could make it true.
The first man who walks down the street will be my one love and I will be true to him as long as he’s true to me.
She turned around twice and held her breath as a way to seal the bargain. She wore her favorite shoes, ones her father had bought her in Connecticut, leather slippers so light she felt as though she were barefoot. Her red hair reached her waist. She has seventy-four freckles on her face – she had counted – and a long, straight nose her father had assured her was elegant rather than large. She watched the sky darken. There was a line of ashes up above, a sprinkling of chimney soot. Perhaps her father was up there, watching over her. Perhaps he was knocking on his casket, begging to be let out. Or maybe he was here with her still, in her heart, making it difficult for her to breath whenever she thought about her life without him. Arlie felt her aloneness inside her, but she was hopeful, too. The past was done with. Now she was made out of glass, transparent and clear. She was instant in time. One damp evening, two stars in the sky, a line of soot, a chattering gathering of neighbors who barely knew her in the dining room. She had convinced herself that her future would arrive on the street where she’d lived her whole life if only she’d wait long enough. If she trusted in fate.

Dialah istri pertamanya. Dahulu saat jumpa pertama, umurnya baru tujuh belas tahun. Arlyn Singer, gadis yang waktu itu sedang berdiri di beranda depan rumahnya di suatu sore. Baginya waktu serasa telah berhenti. Ayahnya baru saja meninggal; acara makan malam dalam suasana berkabung pun baru saja selesai beberapa jam sebelumnya. Beberapa orang tetangga berkumpul di rumah duka itu. Mereka duduk muram mengelilingi sebuah meja makan kayu mahoni kokoh yang sudah lebih dari sepuluh tahun tak pernah dipakai. Sekarang, panci-panci berisi makaroni keju dan kue red velvet serta baki besar penuh buah-buahan berjajar di atasnya. Makanan-makanan itu cukup untuk persediaan Arlie selama sebulan, namun ia sama sekali tak berselera.
Ayah Arlyn adalah seorang kapten kapal feri, dialah satu-satunya orang yang paling berarti dalam kehidupannya, terutama di tahun-tahun terakhir masa hidupnya. Meski raganya digerogoti penyakit, namun kepribadiannya semakin bercahaya layaknya bintang yang bersinar di langit kelam. Ia adalah seorang lelaki yang pendiam dan suatu ketika ia mulai suka bercerita. Ada sebuah legenda, konon ada batu yang suka muncul di kegelapan menyerupai tebing karang misterius yang sering mengaramkan kapal-kapal feri. Ia bercerita, dari semua korban kapal karam itu, ia tahu persis siapa saja yang takkan pernah kembali. Dia menggambar suatu gugusan bintang yang bisa menjadi penunjuk arah pulang bagi orang yang tersesat menggunakan sebatang krayon merah. I juga bercerita tentang suku yang tinggal di seberang sungai di bagian Connecticut sana. Suku yang dapat menumbuhkan sayap bila menghadapi bahaya. Mereka tampak seperti orang biasa. Namun bila saatnya tiba, ketika kapal mulai karam atau diamuk api, mereka akan menampakkan wujud asli mereka untuk dapat menyelamatkan diri.
Di atas meja kecil di samping tempat tidurnya, ada batu-batu koleksi. Konon batu-batu itu pernah berada di dalam perutnya sewaktu muda dulu ketika terjadi peristiwa kapal karam. Dialah satu-satunya korban yang selamat dari musibah itu. Saat kejadian, ia sedang berdiri di atas dek kapal dan beberapa saat kemudian tiba-tiba ia sudah melayang di awang-awang, lalu jatuh terhempas ke atas gelombang arus menuju Connecticut dengan mulut dan lambung dipenuhi batu.
Tatkala dokter datang untuk menyampaikan bahwa kapten sudah tak punya harapan lagi, mereka malah merayakannya dengan minum bersama. Di dalam gelas-gelas wiski mereka, kapten tidak memasukkan es tetapi malah memasukkan batu.
Untuk keberuntungan Anda, katanya kepada dokter. Yang aku inginkan hanyalah kebahagiaan putriku. Itulah satu-satunya keberuntungan yang kuharapkan.
Arlyn terisak-isak di sisi ranjang ayahnya, berharap ayahnya kembali, namun apa daya, bukan dirinya yang menentukan hidup ayahnya. Ia sedang mengingat-ingat nasehat sang kapten dalam kalimat terakhirnya; masa depan adalah sebuah negeri asing dan tak terduga, Arlyn harus siap menghadapi apapun yang akan terjadi. Semenjak ayahnya terbaring sekarat, hidupnya selalu diliputi kesedihan. Sekarang, tubuhnya terasa ringan karena ia tak tahu lagi untuk apa hidup di dunia ini. Begitu ringan hingga semilir angin pun bisa saja menerbangkannya ke angkasa malam menembus jagad raya.
Arlyn menumpukan tangannya pada birai beranda dan mencondongkan tubuhnya ke atas kuncup-kuncup bunga Azalea merah dan merah jambu. Terlepas dari keadaannya saat ini, Arlyn sebenarnya adalah anak yang optimis. Ia masih sangat muda namun ia tahu persis bahwa kehidupan itu bukanlah gelas yang setengah terisi atau setengah kosong, namun laksana gelas indah yang siap diisi apa saja. Ia membisikkan sebuah penawaran, membisikkannya dengan penuh keyakinan agar penawaran itu bisa menjadi kenyataan.
Pria pertama yang melewati jalan itu akan menjadi cinta sejatiku. Aku akan selalu setia kepadanya selama ia menjaga kesetiaannya padaku.
Ia berputar dua kali lalu menahan napas seolah-olah menyegel penawaran itu. Ia mengenakan sepatu kesayangan yang dibelikan ayahnya di Connecticut, yaitu sepasang selop kulit yang sangat ringan hingga saat dipakai terasa seperti bertelanjang kaki. Rambut merahnya terurai hingga ke pinggul. Ada tujuh puluh empat bintik di wajahnya – ia pernah menghitung. Ia merasa hidungnya terlalu besar meski ayahnya selalu meyakinkan dirinya dengan mengatakan bahwa hidungnya yang panjang dan lurus itu sangat serasi. Ia memandangi langit yang beranjak gelap. Tampak taburan abu jelaga cerobong asap yang menggaris di udara. Mungkin ayahnya ada di atas sana sedang mengawasinya. Mungkin sedang mengetuk-ngetuk peti matinya minta dikeluarkan. Atau mungkin malah berada di sini bersamanya, di hatinya. Dadanya terasa sesak setiap kali membayangkan hidup tanpa ayahnya. Di dalam hati, Arlie merasa sendirian, namun ia merasa masih punya harapan. Yang lalu biarlah berlalu. Seketika, ia melihat masa depannya laksana kaca yang jernih dan tembus pandang. Di sebuah sore, ketika udara terasa lembab, ditemani dua bintang berkelip di langit yang berhias garis jelaga dan semua tetangga yang berkumpul dan bercengkerama di ruang makan yang mungkin sama sekali tak mengenalnya, ia membuat ketetapan hati bahwa masa depannya akan tiba di jalan depan rumah tempat ia menghabiskan seluruh hidupnya itu. Ia harus percaya takdir, hanya perlu sedikit bersabar.
