Voorbeeld van een probleem, oplossing en eindresultaat:
Een bedrijf of instantie heeft een helpdesk waar klanten naar toe kunnen bellen. Dit informatienummer van de helpdesk is echter altijd overbezet. De wachttijden zijn onredelijk lang, waardoor bellers geïrriteerd reageren zodra ze iemand aan de lijn krijgen. Ook worden er schriftelijke klachten ingediend over de slechte bereikbaarheid die door een apart team worden behandeld. Het probleem is hier dat de overbezetting van het informatienummer zowel irritatie bij klanten oplevert als extra werk voor het bedrijf. Het bedrijf realiseert zich, na onderzoek, dat de helpdeskmedewerkers vaak dezelfde vragen krijgen. Example of a problem, solution and result:
A company has a help desk to take calls from customers. The waiting time to get through to the helpdesk though is unreasonably long, and customers respond irritated by the time they get to a support assistant. Customers also file written complaints about how hard it is to reach support, which are processed by a separate team. The problem is that the high call load to the help desk causes irritated customers, and extra work for the company. Investigation shows that support assistants at the help desk often get the same questions from customers. A possible solution for this problem is creating a website that answers frequently asked questions (faq’s).

Phase I Trials Demonstrate Safety of Kahalalide F in Prostate Cancer Patients
PharmaMar has presented Phase I and Pharmacokinetic results of Kahalalide F at the 40th Annual Meeting of the American Society of Clinical Oncology (ASCO), taking place in New Orleans from 5-8 June.
A poster presented by Dr. Jos H. Beijnen et al, The Netherlands Cancer Institute, detailed the results from: "A Phase I and Pharmacokinetic study of Kahalalide F given intravenously over one hour, during five consecutive days every three weeks in patients with advanced or metastatic androgen refractory prostate cancer."
The objectives of the study were to determine the safety and maximum tolerated dose, explore the pharmacokinetics and document the anti-tumour activity of Kahalalide F. The setting was patients suffering from androgen independent prostate tumours. Thirty-two patients were registered with a median age of 68 years. They had metastases to the bone, lymph nodes, liver and/or lungs and had received prior hormone therapy, radiotherapy and/or surgery.
Overall, hematologic toxicities were negligible and were not clearly dose-related. Using the NCIC-CTC (National Cancer Institute of Canada - Common Toxicity Criteria) classification, G1 anemia, thrombocytopenia and neutropenia were observed in 22 patients. G2 anemia and neutropenia occurred in 8 patients. There were no G3 or G4 hematologic toxicities. Other side effects observed were nausea & vomiting, fatigue, hematuria and hypersensitivity reactions, none of which were at G4. The major drug related toxicity was rapidly reversible transaminase elevation at G3/G4.
The conclusion of the study was that Kahalalide F can be administered safely as a 1-hour intravenous infusion during five consecutive days at a dose of 560 mg/msquared/day once every three weeks. Future studies in hormone refractory advanced prostate cancer are warranted.
Dr Miguel Angel Izquierdo, Director of Clinical Development at PharmaMar, said:
"These results are consistent with previous studies undertaken with Kahalalide F. We are impressed by the safety profile of the product and are on track to continue the development of this compound."

Op de 40e jaarlijkse bijeenkomst van de American Society of Clinical Oncology (ASCO) in New Orleans van 5 tot 8 juni heeft PharmaMar de resultaten van het fase I-onderzoek en de farmacokinetische gegevens van Kahalalide F bekend gemaakt.
Een poster, gepresenteerd door Dr. Jos H. Beijnen et. al. van het Nederlands Kankerinstituut, laat de resulaten zien van "Fase I en farmacokinetisch onderzoek naar Kahalalide F, gedurende 1 uur intraveneus toegediend op vijf achtereenvolgende dagen, en driewekelijks herhaald, bij patiënten met gevorderd of gemetastaseerd hormoonrefractair prostaatkanker."
Het doel van de studie was de veiligheid en de maximaal getolereerde dosis van Kahalalide F vast te stellen, de opname, verwerking en uitscheiding ervan te onderzoeken, en de effectiviteit te documenteren. Het onderzoek werd gedaan onder patiënten met hormoonongevoelig prostaatkanker. Tweeëndertig patiënten met een mediane leeftijd van 68 jaar namen aan het onderzoek deel. De patiënten hadden uitzaaiingen naar het bot, de lymfeklieren, de lever en/of de longen, en waren reeds behandeld met hormoontherapie, bestraling en/of geopereerd.
Het onderzoek toonde aan dat de hematologische toxiciteit verwaarloosd kon worden, en niet duidelijk met de dosis samenhing. Gebruikmakend van het classificatiesysteem van het Nationale Kankerinstituut in Canada (NCIC-CTC - National Cancer Institute of Canada - Common Toxicity Criteria), werd bij 22 patiënten G1-anemie, trombocytopenie en neutropenie vastgesteld. G2-anemie en neutropenie kwam voor bij 8 patiënten.
Er werd geen G3 of G4 hematologische toxiciteit gevonden. Andere bijwerkingen, zoals misselijkheid en braken, vermoeidheid, hematurie en overgevoeligheidsreacties bereikten geen G4-niveau. De belangrijkste druggerelateerde toxiciteit was een snel-reversibele transaminase op het niveau van G3/G4.
De conclusie van de studie was dat Kahalalide F veilig in de vorm van een 1-uur durend infuus van 560 mg/m²/dag gedurende 5 achtereenvolgende dagen kan worden toegediend, en iedere drie weken kan worden herhaald. Toekomstig onderzoek naar hormoonrefractair gevorderd prostaatkanker is aanbevolen.
Dr Miguel Angel Izquierdo, Directeur Klinische Ontwikkeling bij PharmaMar, merkte op: "De resultaten zijn in overeenstemming met eerdere onderzoeken naar Kahalalide F. Wij zijn onder de indruk van het veiligheidsprofiel, en bezig deze stof verder te ontwikkelen.”
