PHARMACOLOGICAL PROPERTIES
Pharmacodynamic Properties
Olanzapine is an antipsychotic, antimanic and mood regulating product that affects on a group of receptors. Olanzapine shows affinity for 5HT 2A/2C, 5HT3, 5HT6, dopamine D1, D2, D3, D4, D5, cholinergic muscarinic M1-M5 receptors; alpha adrenergic and histamine H1 receptors (Ki>100nM). 5HT, dopamine and cholinergic antagonism were shown complying with receptor binding profile in animal studies. Affinity of olanzapine for serotonin 5HT2 receptors is higher tna for dopamine D2 receptors. Electrophysiologic studies showed that olanzapine selectively decreases dopaminergic neuron stimulation and has a very low effect on striatal paths efficient on motor functions. Olanzapine decreases the conditioned aversion response which is an indicator for antipsychotic efficiency in doses under levels that reveal catalepsy, a symptom of motor side effects. Unlike from some other antipsychotics, olanzapine increases the response to anxiolytic tests. It was shown that olanzapine regulates either positive or negative symptoms of schizophrenia significantly.
Pharmacokinetic Properties
Absorption
Olanzapine is well absorbed following oral use and peak plasma concentration is obtained within 5-8 hours. Absorption is not affected by food. Oral bioavailability is about 57%.
Distribution
In 7-1000 ng/ml concentration, olanzapine plasma protein binding is 93% (mainly albumin and alpha 1- acide-glucoporotein)
Elimination
Olanzapine is metabolized by conjugation and oxidation in liver. First pass elimination is 40%. Basic metabolite in the circulation is 10-N-glucuronide which can not pass through the blood-brain barrier. Cyctochrome P450 CYP 1A2 and CYP 2D6 enzymes provides formation of N-desmetil and 2-hydroxymetil metabolites. Both of these metabolites showed lower in vivo pharmacological efficiency than olanzapine in animal studies. Olanzapine is responsible from basic pharmacological efficiency.
