Anwendung von cortico-spongiösen Blöcken und gleichzeitiger Sinusbodenelevation mit Hilfe der3D-CT-Block-Technik

Die Verwendung von Knochenimplantaten als Rekonstruktionstechnik gewinnt zunehmend an Bedeutung. Zahlreiche Autoren haben in den letzten Jahren Augmentationen mit Knochenimplantaten alloplastischer Herkunft beschrieben [1-11]. Die Verwendung dieser Technik bietet den Vorteil, daß der Patient nicht einem Eingriff zur Entnahme des Implantats unterzogen werden muß (Kinn oder Rahmen) und daß es keine quantitativen Einschränkungen in bezug auf das für die Rekonstruktion erforderliche Material gibt, da es kommerziell erhältlich ist.
Die Computertomographie (CT) ist heute in der Implantologie zu einer Routineuntersuchungsmethode geworden, da sie die einzige Methode ist, die signifikante Daten zur Qualität des Knochens, zur Morphologie der anatomischen Strukturen und zur dreidimensionalen Darstellung der Kieferknochen liefern kann [1, 12-15]. Die Verarbeitung der CT-Daten mit einer speziellen Software (Vimplant, Cybermed) erlaubt es auch am PC 3D-Rekonstruktionen der Kieferknochen zu erstellen. Geeignete Servicezentren können diese 3D-Dateien in feste Modelle aus Kunststoff umwandeln, die den anatomischen Gegebenheiten zuverlässig entsprechen.
Materialien und Methoden
Bei einer Patientin sollte eine Rekonstruktion mittels eines cortico-spongiösen Blockes (Tutodent® CS-Block, Tutogen Medical GmbH) durchgeführt werden. Auf Verordnung wurde eine CT-Aufnahme erstellt und auf einem Datenträger gespeichert. Die axialen Bilder der Kieferknochen wurden mit der Software Vimplant™ bearbeitet.
Es wurde eine virtuelle Planung des Falles vorgenommen. Danach wurde eine Datei im *.stl-Format („Solid to Layer“) extrapoliert. Die Datei ermöglicht es, mittels Sintermaschinen feste Modelle durch schnelle Prototypherstellung zu reproduzieren. Das für die Modelle verwendete Material ist Duraform™ (DuraForm PA, 3-D Systems, Valencia, CA), ein gegen hohe Temperaturen resistenter Kunststoff (Deformation bei 184°).
Anhand der CT-Daten wurde der zu erstellende cortico-spongiöse Block, im vorliegenden Fall eine horizontale Rekonstruktion der Zähne 24 und 25, und eine Sinusbodenelevation der Kieferhöhle nach der Zahnleistentechnik von Dr. F. Cosci geplant. Am festen Modell in Duraform™ wird das Einsetzen des cortico-spongiösen Blocks in die endgültige Position simuliert, die dabei für die prothetische Versorgung am besten geeignet gehalten wurde. Am Modell werden zwei Demonstrationsimplantate eingesetzt. Mit einem Filzstift wird die Basis des knöchernen Lagers der chirurgischen Schablone gezeichnet. Der Zahntechniker modelliert anhand der Angaben die chirurgische Schablone. Anschließend wird die Schablone aus Harz in Folie versiegelt und autoklaviert.
Das erstellte Kiefermodell wurde ebenfalls in Folie versiegelt und mit den üblichen Sterilisationsprogrammen im Autoklaven sterilisiert. Der Operationssaal wurde unter sterilen Bedingungen vorbereitet, wie sie bei implantologischen Eingriffen oder bei Knochenimplantationen angewendet werden. In dieser Umgebung wurde der cortico-spongiöse Block am vorher sterilisierten 3D-Modell angepaßt, wobei stets alle Vorschriften des sterilen Arbeitens eingehalten wurden, die während eines Eingriffs befolgt werden müssen.
Dieses Vorgehen bietet zahlreiche Vorteile:
• Die Möglichkeit, an einem sterilen und stets gut sichtbaren Ort zu arbeiten, da dieser frei von Blutungen und Mundflüssigkeiten ist.
• Das Modell kann zur Kontrolle der Genauigkeit der Modellierung des corticospongiösen Blocks gedreht werden.
• Es kann die gesamte für die Modellierung erforderliche Zeit aufgebracht werden ohne die Befürchtung, daß die Wirkung der Anästhesie nachläßt.
• Der Patient braucht nicht psychologisch geführt zu werden (autologe Blockent- nahme aus dem Kinnbereich oder Rahmen).
Nach Abschluß der Präparation des corticospongiösen Blocks und deren Fixierung am Modell mittels geeigneter Osteosyntheseschrauben wird das Ganze unter umsichtiger Schonung der auf dem Modell angebrachten cortico-spongiösen Blöcke mit der Technik der doppelten sterilen Verpackung eingepackt: Ein Beutel wird mit der offenen Seite in einem anderen Beutel sterilisiert. Der unsterile Assistent öffnet den sterilen Beutel, der den sterilen Beutel enthält, und der sterile Assistent nimmt den sterilen Beutel mit der offenen Seite und führt das angefertigte Produkt in diesen ein; danach wird die offene Seite des Beutels versiegelt. Mit dieser Methode ist es möglich, das Modell mit den angepaßten cortico-spongiösen Blöcken aufzubewahren. Die eigentliche chirurgische Phase, die aufgrund der Anforderungen der Praxis und des Patienten geplant wird, wird bedeutend kürzer sein als wenn dieselbe Art des Eingriffs in herkömmlicher Weise durchgeführt wird. Die deutlich verkürzten Operationszeiten garantieren dem Patienten einen angenehmeren postoperativen Verlauf mit weniger Ödemen und Schmerzen.
Diskussion
Die Ziele dieser Technik sind:
• Drastische Verkürzung der Operationszeiten am Patienten, bei dem eine Rekonstruktion mittels xenogenen Knochenblöcken durchgeführt werden soll.
• Verbesserung der Qualität der Knochenblöcke, da die Präparation am 3D-Modell eine optimale Kontrolle der Modellierung und eine Überprüfung der durchgeführten Arbeit ermöglicht.
• Verkürzung der Operationsphasen: Implantation, Einsetzen und Anhebung der Kieferhöhle werden in einer Sitzung durchgeführt.
Dank der am Modell durchgeführten Planung, der Anfertigung der chirurgischen Schablone für das knöcherne Lager und der Modellierung der Knochenblöcke mit der3D-Block-Technik werden die Standardverfahren für Knochenblöcke erheblich vereinfacht. Es ist jedoch stets zu berücksichtigen, daß mit dieser Methode zwar die Operationszeiten drastisch verkürzt und die Fehlermöglichkeiten deutlich verringert werden, dennoch bleibt die Tatsache bestehen, daß wie bei allen regenerativen Techniken die Schonung des Weichteilgewebes eine grundlegende Rolle spielt, und daß der Operateur daher in der Lage sein muß, dieses so gut wie möglich zu schonen, um einen Behandlungserfolg zu erzielen.
Zusammenfassung
Mit Hilfe von CT-Daten lassen sich mit dem Material Duraform™ dreidimensionale Kiefermodelle erstellen. Dieser temperaturresistente Kunststoff ist sterilisierbar. Unter sterilen Bedingungen wurde anschließend ein cortico-spongiöser Block (Tutodent® CS- Block, Tutogen Medical GmbH) modelliert und an die Oberfläche der Kiefermodelle angepaßt. Nach Abschluß der Vorarbeiten werden die Modelle mit dem cortico-spongiösen Block, Titan-Fixationsschrauben und der Kollagen Membran (Tutodent® Membran, Tutogen Medical GmbH) erneut in einem sterilen Beutel verpackt und sterilisiert. Zum eigentlichen Patienteneingriff wurde der cortico-spongiöse Block, Titan- Fixationsschrauben und die Kollagen- Membran vom Modell entfernt und im Patienten eingesetzt.
Diese 3D-CT-Block-Technik erlaubt es, die Operationszeiten am Patienten drastisch zu verkürzen, da der längere Teil der Arbeit am 3D-Modell durchgeführt wird; außerdem hat man eine bessere Kontrolle über das Fitting des cortico-spongiösen Blocks, die nach trockener Modellierung von mehreren Blickwinkeln aus ohne sichtbehindernde Hindernisse wie Blutungen, Läppchen und die Begrenzungen durch die Mundhöhle überprüft werden können.
Schlußfolgerung
Die Technik erwies sich als sehr einfach und die Übereinstimmung zwischen dem festen Modell und der Kieferanatomie als klinisch mehr als zufriedenstellend.

Use of cortico-cancellous blocks with simultaneous elevation of the floor of the maxillary sinus, with the aid of the 3D-CT block technique

The use of bone implants for reconstructions is becoming increasingly important. Numerous authors have described augmentations using bony allograft implants in recent years [1-11]. The use of this technique has the advantage that the patient does not need to undergo an operation to harvest the implant (from chin or body of mandible) and that there are no restrictions on the material needed for the reconstruction, since it is commercially available.
Computerised tomography (CT) has become a routine technique in the field of implantation, since it is the only method that can provide significant data on bone quality and the morphology of anatomical structures, and offer a 3D representation of the bones of the jaw (1, 12-15]. The processing of CT data with specially designed software (Vimplant, Cybermed) even makes it possible to carry out 3D reconstructions of the bones of the jaw on a PC. Specialist service centres can convert these 3D files into solid plastic models which correspond reliably with the anatomical features.
Materials and Methods
A reconstruction needed to be carried out on a female patient using a cortico-cancellous block (Tutodent ® CS Block, Tutogen Medical GmbH). A CT scan was requested and stored on digital media. Axial images of the jaw bones were processed with Vimplant™ software. A virtual treatment plan was undertaken. A data file in *.stl format (“Solid to Layer” was then derived. With this file, a solid model can be produced using rapid prototype manufacturing with a sintering machine. The material used for these models is Duraform™ (DuraForm PA, 3-D Systems, Valencia CA), a plastic that is resistant to high temperatures (deforms at 184°).
On the basis of the CT data, the required cortico-cancellous block, in this case a horizontal reconstruction of teeth 24 and 25, and an elevation of the floor of the maxillary sinus was planned according to the dental lamina technique of Dr F Cosci. The implantation of the cortico-cancellous block was simulated on the solid Duraform™ model in the final position expected to give the best prosthetic result. Two demonstration implants were inserted into the model. The base of the bony bed of the surgical template is outlined with a felt-tip pen. The dental technician models the surgical template on the basis of the data provided. The resin template is then sealed in foil and autoclaved.
The prepared jaw model was also sealed in foil and autoclaved using the normal sterilisation program.
The operating theatre was prepared under sterile conditions, as used in implant procedures or bone implants. In this environment, the cortico-cancellous block was fitted to the previously sterilised 3D model while strictly following those sterile techniques that need to be followed in such cases.
This procedure offers numerous advantages:
• The ability to work in a sterile and clearly visible operation field, free from bleeding and oral fluids.
• The model can be rotated in order to check the precision of modelling of the cortico-cancellous block.
• As much time as necessary can be taken for the modelling without concerns that the effect of the anaesthetic will wear off.
• The patient does not need psychological support (removal of autologous blocks from chin region or body of mandible).
After preparation of the cortico-cancellous block and its fixation to the model with appropriate synthetic bone screws, the whole preparation is placed in a double sterile package, taking care to protect the cortico-cancellous blocks on the model: one pack is sterilised with the open side in another pack. The non-sterile assistant opens the sterile pack that contains the other sterile pack, and the sterile assistant removes the sterile pack with the open side and puts the preparation into it; the open side of the pack is then sealed. With this technique the model with the fitted cortico-cancellous blocks can be stored. The actual surgical phase, which has been planned with the requirements of the practice and the patient in mind, will be significantly shorter than if the same kind of procedure were carried out in the usual way. The markedly shortened operation time guarantees a better recovery for the patient, with less oedema and pain.
Discussion
The aims of this technique are:
• Drastic reduction of the operating time for patients requiring a reconstruction with xenogenic bone blocks.
• Improvement of the quality of bone blocks, since preparation on a 3D model allows optimal control of modelling and checking of the completed work.
• Reduction in the number of separate phases of the operation: implantation, fixation and elevation of the maxillary sinus can be carried out in one sitting.
Thanks to planning on the model, the preparation of the surgical template for the bony bed, and the modelling of the bone blocks using the 3D block technique, the standard procedure for bone blocks is significantly simplified. However, it must still be recognised that although the operating time is drastically shortened and the possibility of errors is markedly reduced with this method, the fact remains that as with all regenerative techniques the preservation of soft tissue plays a fundamental role. The operator must therefore protect these soft tissues as well as possible in order to achieve a successful result.
Summary
Three-dimensional models of the jaw can be made out of Duraform™ on the basis of CT data. This temperature-resistant plastic can be sterilised. Under sterile conditions, a cortico-cancellous block (Tutodent ® CS-Block, Tutogen Medical GmbH) was modelled and fitted to the surface of the jaw model. Once these preparatory steps had been completed, the models with the cortico-cancellous block, titanium fixation screws and the collagen membrane (Tutodent ® Membrane, Tutogen Medical GmbH)* are placed in a sterile pack and sterilised. At the actual operation the cortico-cancellous block, titanium fixation screws and the collagen membrane are removed from the model and implanted in the patient.
This 3D CT block technique makes it possible to drastically shorten operating time for the patient, since the greater part of the work is carried out on the 3D model. In addition one has better control over the fitting of the cortico-cancellous blocks, which can be checked after dry modelling from various viewpoints without visual impediments such as bleeding, swabs, and the borders of the oral cavity.
Conclusion
The technique proved to be very simple, and the correspondence between the solid model and the anatomy of the jaw was more than satisfactory from a clinical point of view.
