Magnetic Resonance Imaging (MRI) examination while pregnant
I am aware that there are no documented instances of complications or harmful effects in pregnant patients who have undergone MR scans, no comprehensive studies have been undertaken to determine the effects on the fetus of either the magnetic field or radio waves utilized to perform the examination.
By signing this consent form, I agree that XXXX Radiology, its employees, or any radiologist associated with the facility shall not be held responsible for any complications encountered before, during, or after childbirth to either mother or baby.
I also confirm that my physician explained there was no alternative diagnostic test available to me, and my physician recommended that I undergo a MRI exam.

Bingo 75 Balls (USA)
The game is played with several others and its aim is to match numbers of a given card which appears as a 5X5 matrix and are unique to each player with numbers randomly drawn out and called between 1 and 75 to form a specific given pattern. The first player to complete the given pattern on their card with the numbers that have been called, shouts bingo and win the pot of money generated by all the bingo cards purchased for that game. The first prize will of course differ greatly depending on the number of players in the game.
To enter the game, customers must purchase cards containing a grid consisting of 24 numbers in a 5x5 format. The word bingo is printed on the top of the card and each letter represents a column containing a random set of numbers as follows: under the column with the letter “B” the numbers range from 1 to 15, under “I” from 16 to 30, under column “N” 31 to 45, under column “G” 46 to 60 and finally under “O” 61 to 75. Each card provides a unique set of numbers generated by a complex mathematical calculation allowing a sophisticated permutation.

Se juega entre varias personas y su objetivo consiste en conseguir que todos los números de un determinado cartón coincidan. Los cartones constan de 25 casillas organizadas en 5 columnas y 5 filas. Cada cartón es único, es decir, que no coincide con los cartones de los demás jugadores, y contiene números del 1 al 75 escogidos al azar que forman un patrón específico. El primer jugador en completar el patrón de su cartón con los números que hayan salido, grita bingo y gana el bote generado por la compra de todos los cartones de esa partida en concreto. Por supuesto, el primer premio variará mucho, en función del número de jugadores que participen en cada partida.
Para participar en el juego, los jugadores deben comprar cartones con 24 números distribuidos en una cuadrícula de 5x5 casillas. En la parte superior de cada cartón figura la palabra “bingo” y cada letra de dicha palabra representa una columna con una serie de números escogidos al azar. En la columna con la letra “B” figuran los números del 1 al 15, en la columna “I” del 16 al 30, en la columna “N” del 31 al 45, en la columna “G” del 46 al 60 y, por último, en la columna “O” del 61 al 75. Cada cartón tiene una serie de números única, generada mediante complicados cálculos matemáticos que permiten una permutación muy sofisticada.
