嗨大家好！
我有个美国客户，最近要求我填写上述表格。听说主要跟税务有关。
我对美国税制，对新加坡和美国是否有任何条约上的税务优惠都不清楚，所以颇有点老鼠拉龟的感觉。
具体来说，我想请教如何填写第７号到第１０号。如果哪位大侠肯指教，并需要该表格参考，小弟马上给您寄上PDF文档。

（题外话：个人觉得美国英汉翻译的价格还不错，远超类似US$0.05/字的超低价，不知各位前辈以为如何？
