Requests for BB rating don't specify agency name in header

Lately, notification e-mails about requests to rate a specific agency in the Blue Board do not include the name of the agency in the header. The current header for all requests is: "[ProZ.com Blue Board] Request for Information".

It would be nice if the agency name were included again.
