"Unknown exception" when trying to 'create translation'

I have completed a translation and went to 'create it' in SDLX (2007), but an error sign comes up saying "Unknown exception". I have never seen this before and the only reference I have found online is for opening files. Ideas?

Improper source file Dec 14, 2010

I met with this situation when the source file format was incomplete e.g. file after many conversion or many languages. I open source file again with T2009 or TagEditor. It can be possibly remedied. [for doc file, I saved as rtf and opened in SDL Edit again.]
