Insurance promoted by The Institute of Linguists (UK) - why are certain things not covered?

I have some questions about the insurance offered in conjunction with the IOL (UK institute of linguists). I can't get through to them on the phone and can't find the answers in their FAQs.

First of all, the insurance offered seems to be professional indemnity insurance only and does not include any health insurance. Am I wrong on this or must health insurance be sourced independently? Or maybe many people see it as being unnecessary?

I have also noticed that it does not cover work from US or Canadian companies which is surely not great. I would imagine that a fair amount of my work may well come from US or Canadian companies. Does everyone buy extra insurance from the same people to cover this or from a different company? Or just live with the risk?

Although I can't comment on the IoL scheme as I took out my insurance through the ITI's arrangements, I can say that my ITI cover was slightly more expensive than it would otherwise have been because of the percentage of my income over the past year that came from North America, which was over the 10% threshold. So I was still able to get cover, but clearly North American clients are regarded as carrying a higher risk, presumably because of the "sue culture" that exists in that region (not that it doesn't exist here, albeit perhaps to a lesser degree!) Whether or not the IOL scheme can do the same for you, I don't know. Perhaps you could contact the insurer directly to find out, there certainly used to be a link to the insurer's website on the IOL's website but I haven't checked lately.

I've never heard of a professional indemnity scheme for translators that also includes health cover, they're two separate things as far as I know.
