Good morning, everybody!

Does anyone know where I can find the form 2040 in german language??? I already looked on their website and I found quite a lot of forms but not this one...
Unfortunately, it's quite urgent...

bist Du sicher, daß es das Formblatt überhaupt gibt? Das EPO kennt das Formblatt in keiner Sprache. Wofür soll es sein?

I can't find any reference at all to an EPO Form with this number. Is the number definitely correct? Could you tell us what the Form is about or perhaps its title?

Hi Alison,

the number ist perfectly correct! In the meantime I got the information that this is an internal form and therefore I cannot have a look at it in German... By now I have translated and delivered the text.
