Physiologiquement, il existe un décollement virtuel des deux feuillets péricardiques en systole, (ce décollement correspond au liquide intra-péricardique physiologique). L'épanchement est dit mineur lorsque le décollement péricardique est inférieur à 1 cm. L'épanchement est dit de moyenne abondance lorsque le décollement est compris entre 1 et 2 cm d'épaisseur. Enfin l'épanchement est considéré comme très volumineux lorsque le décollement est supérieur à 2 cm d'épaisseur. Ces épanchements sont rarement bien tolérés. Il peut exister à ce stade des signes de compression des cavités droites. It remains the most sensitive technique for the detection of this disorder. Around the heart there exists a potential cavity between the visceral and parietal layers of the serous pericardium, into which the heart is invaginated during its development. There is normally a little fluid in this cavity that acts as a lubricant for heart movement, but the two layers of the serous pericardium essentially remain in contact.
