German to English translations [PRO]
Medical - Medical (general) / clinical trials
German term or phrase: Prüfbögen
A description of a company responsible for monitoring of clinical trials:

"Bisher durchgeführte klinische Arzneimittelstudien in den therapeutischen Einsatzgebieten Datenerfassung in Prüfbögen und Transfer der Daten mittels Fax, Modem und Internet."

This is followed by a list of medical fields (gastroenterology, hematology, etc.)

I am looking for the 'official' technical term. Thanks!

Linguee seems to favour "case report form" or "check list".

Here's an English-German KudoZ query which would seem to support "case report form":

agree  MMUlr: ... and on the German plural: *Bogen* (instead of Bögen) is the first choice, see Duden ref.: Bo|gen, der; -s, - u. (bes. südd., österr. u. schweiz.) Bögen ... - showing that Bögen is a more "southern" variation.
