I'm translating a chart from a health insurance plan of US. In the first column there are several benefits/services listed and the second column shows the amount you have to pay for the benefit.

Emergency room $100 copayment then deductible then 20%

Inpatient Services Deductible then 20%
/Outpatient Surgery

I don't get the meaning of the phrases in the second column, but mostly the meaning of "then": "y" or "luego".

• Al comienzo de la página se enumeran los montos deducibles/las franquicias/los deducibles/etc. según quieras llamarlos. Esta es la cifra que el afiliado o miembro asegurado debe pagar de su propio bolsillo antes de que el seguro comience cubrir sus gastos médicos.

• Para el caso de la sala de emergencias:

– establecen un pago de $100 solamente por concurrir
   – luego no cubren nada hasta que hayas alcanzado el deducible
      (esto incluye todos tus gastos médicos)
   – a partir de ese momento, tendrás que pagar el 20%

• Según indican, si vas a la sala de emergencias en un hospital de la red y te internan, te perdonan los $100 ("copayment waived if...").

Para hacerlo menos literal: "copago de $100; se aplica el monto deducible y luego el 20% (de los gastos)".

Agustina, you would say "despues." It means that the person who has that plan would have to pay the deductible, then 20% of the cost of whatever medical services s/he receives.
