Can anybody confirm the meaning of pc here? I think it is 'perte de conscience'.

Could be "poste de contrôle" or perte de conscience". If pc is in front of "pour" it is more likely "poste de contrôle".

If it says diplopie it's diplopia. If it says double vision it's double vision. No reason to downgrade a good medical term just to enter another answer.
I have never seen pc for "pour consultation" in my 25 years at hospital.
