pet vaccination record book


Explanation:
This is a booklet that every pet owner has. It's used to keep a note of the dog's and owner's details (name, microchip number etc.) and every time the pet is vaccinated, a sticker is entered in the book with the date, lot number of the vaccine and so on. Owners can also, at their discretion, add other details such as any surgery the pet has had. However, vaccination details are mandatory if you want to take your dog to certain places, enroll them in classes or whatever. So although this booklet can potentially serve a wide range of uses, "vaccination record book" is what it essentially is.
