Assuming this refers to the SÄS Hotell at Borås (see reference), I think that ’permission’ here refers to capable patients being ”signed off” to leave the general wards to stay in this accommodation and look after themselves hotel-wise, while still receiving hospital medical treatment. In which case, I would translate the whole sentence as ” If the patient is formally assigned to SÄS Hotell, the charge includes the cost of medical care and medicines”. (This is in contrast to Out Patients who have to pay for these, up to a maximum limit.) Needless to say this concept is unknown in UK.
