Unicamente la descrizione lascia intendere di che cosa parla:
Se guardi il palmo della mano, lui parla soprattutto delle zone intorno alle articolazioni tra le ossa metacarpali e le prime falangi.
Se guardi il lato palmare soprattutto della prima falange del dito medio, noti che è più elevato degli altri, come una montagnuola (monticulus). Lo spessore di queste "montagnuole" varia sia per un fatto ereditario/familiare, che per il tipo di manualità dell'individuo. Qui la causa sembra essere un carico anomalo dei muscoli e tendini della mano a causa di un problema di innervazione o ortopedico di altra natura (incidente...ecc.).
