Hi,

The patient was hospitalized for decompensated heart failure.
In the history I have "Pas de syndrome infectieux. Pas d'oublie de prise de thérapeutique. Pas d'écarts alimentaires."

Does this mean "no changes to diet"? "Écarts" seems stronger in French.

Another option: No non-adherance to doctor's dietary instructions.
More formal language.

Obviously the person has serious heart problems and has been given strict instructions by his/her doctor. This much is obvious.
