Which one of the three alternatives fits into this context?

The other dictionary says just "mistake, error".

I wonder if it's her mistake or the mistake (?) of another person. If I should make a declaration of this kind and I were aware of the mistake (so I could include it in my declaration), I would correct it before, right?
