In this sense the asker is challenging

Although it can mean both, my answer and Fuad's, the tone of the asker will determine the actual meaning.

"Why doesn't she want me?" Or "why, on Earth, wouldn't she want me?"

Having Syrian roots, I would feel the subtle difference.
